PartⅠ A SURVEY OF THE LONG-TERM CARE NEEDS OF THE ELDERLY IN THE COMMUNITYObjective Through questionnaire survey,investigated the long-term care needs of community disabled elderly,and analyzed the influencing factors of long-term care needs of disabled elderly,Providing reference for the construction of long-term care integration service system for the disabled elderly.Methods Literature research method: the literature research method was used to design and implement the questionnaire on the long-term care needs of the elderly in the community.The questionnaire includes three parts: general information,the elderly ability assessment table and the long-term care demand questionnaire.All the scales have good reliability and validity,All the scales had good reliability and validity.Questionnaire survey method: using the convenient sampling method and the purpose sampling method,210 disabled elderly registered in 11 community health service centers in Tai an were investigated.SPSS 18 was used for data statistics and analysis,including descriptive analysis,t test,ANOVA and multiple linear regression analysis.The difference between P<0.05 and statistical significance was statistically significant.Result The demand for long-term care services of community disabled elderly with different age,education level,marital status,monthly income per capita,living condition and economic source is different.The difference is statistically significant(P<0.05).There was a significant positive correlation between the total score of long-term care demand and the number of chronic diseases and the grade of ability.The number age,educational level,marital status,average monthly income and ability level of chronic disease are the positive factors of long-term care needs,and living conditions and economic sources are negative factors of long-term care needs.The top three levels of impact strength are rank,residence and age in turn.Conclusion The long-term care needs of the disabled elderly are in turn medical care service,spiritual comfort service,daily life care and social participation.The key object of longterm care is the elderly with heavy loss of energy,solitary living,more slow disease,older age and no spouse.The main factors affecting the long-term care demand are the capacity grade and the living condition.The government and relevant departments should take the disabled elderly demand as the guide,according to the different level of disabled elderly,integrated care resources,promoting seamless connection management combining the medical support,establish evaluation standards for long-term care,building suitable for China’s disabled elderly long term care service system.Provide long-term,overall and efficient long-term care services for the disabled elderly.PartⅡ CONSTRUCTION OF LONG-TERM CARE INTEGRATION SERVICE SYSTEM FOR ELDERLY PEOPLE WITH DISABILITYObjective In the survey as the basis,to construct a integrated service system of long term care for the elderly in the community for our loss,through focus group interviews,pre testing,expert group meeting revised,the formation of "disabled elderly community based long-te rm care service system integration" finalized for the disabled to provide practical basis for the management and assessment of long-term care.Methods Based on the survey results of the status,draw on the advantages of long-term care services at home and abroad,and combined with China’s national conditions,to construct a system of draft based on electronic health records,a preliminary revision of the system through focus group interviews,through the system test and pre revision,through expert group meetings,build a loss integrated service system of long term care for the elderly in the community.Result The disabled elderly community-based long-term care service system integration finalized including "main goal","constructing principle","constructing idea","general requirement","service content","organization structure","function structure","operation path" and "service process" 9 aspects of the theoretical framework.A continuous long-term care service for the elderly to provide medical and social resources.Conclusion This system helps to achieve seamless links between home,community,institution and hospital care services.It can provide more scientific,efficient and intelligent long-term care integration services for the elderly,and can be used to improve the current status of evaluation and management of elderly disabled in the community.PartⅢ PRELIMINARY APPLICATION OF LONG-TERM CARE INTEGRATION SERVICE SYSTEM FOR ELDERLY PEOPLE WITH DISABILITYObjective A questionnaire on the cognitive status and satisfaction of the integrated long-term care service integration system for the disabled elderly was designed,and the cognitive evaluation of the long-term care service integration service system for the disabled elderly was carried out to verify the operability and scientificity of the system.Methods Design " disability cognition and satisfaction service integration system of long term care for the elderly in the community" questionnaire and questionnaire,the questionnaire included general information,for the disabled elderly community satisfaction and cognition of long-term care service integration system(the system advantages and disadvantages and difficulties in the implementation of three aspects of the questionnaire)Using the Likert 5 grade scoring method,the higher the score,the higher the satisfaction and the better cognition.By using the convenient sampling method,32 community workers in Tai an,Jinan and Ri zhao were selected for the investigation,and the questionnaire on the cognitive status and satisfaction questionnaire of the integrated system of long-term care service integration system of disabled elderly people was investigated by questionnaire.The survey data were recorded into SPSS 18 statistical software,and the results were analyzed with descriptive statistics and inferential statistics.Result The questionnaire Cronbach ’s alpha coefficient is 0.857 and the reliability is good.It can evaluate the satisfaction and cognition of the community staff on the long-term care service system for the disabled elderly.Community workers have the highest score of systematic superiority in the cognition of community based long term care service system for disabled elderly,which are all above 4 points.The satisfaction of community workers on the long-term care integration service system of disabled elderly people is higher.Conclusion The community staff have better cognition and higher satisfaction to the “long-term care integration service system of the disabled elderly”.The long-term care integration service system of the disabled elderly has some advantages,and there are some disadvantages and difficulties in implementing it.The community based long term care system for disabled elderly has certain adventages.It can optimize service and improve the quality of life of disabled elderly.It is a realistic manifestation of promoting medical reform.We should adopt correspond ding suggestions and Countermeasures to establish and improve the long-term care service system,and actively take care of the long-term care problems of aging.
