Objective:Breast cancer is a hormone-dependent tumor.Studies have shown that the androgen receptor(androgen receptor,AR)other than the estrogen receptor and progesterone receptor is closely related to the occurrence and development of breast cancer. The triple-negative breast cancer,TNBC has the characteristics of high malignancy, high recurrence and metastasis rate, and poor prognosis. However, chemotherapy is still the main treatment. Therefore, research on finding new therapeutic targets is extremely important. At present,AR as a possible new target has been reported in different studies on the relationship between treatment and prognosis of TNBC patients.Currently, the report on relationship between the therapeutic effects and patients’ prognosis of AR and TNBC varies,Thus, this study was to retrospectively analyze the results of immunohistochemistry to detect the expression of AR in TNBC patients with cancer tissue samples, and to analyze the correlation between AR and triple-negative breast cancer patients, such as tumor size and lymph node metastasis, and analyze AR and A possible relationship between the prognosis of triple-negative breast cancer to assess the utility of AR expression in clinically predicting the prognosis of TNBC.Methods:This experiment included 94 patients with TNBC who were admitted to the Department of Breast Surgery, the First Affiliated Hospital of Kunming Medical University from January 2014 to December 2015.Collecting patient’s case and data included tumor size, lymph node metastasis, clinical stage, expression of AR and Ki-67 in immunohistochemistry results, patient age,if has metastasis, and the situation at the time of follow-up.Integrate the data to analyze the correlation between AR expression in TNBC and clinical indicators, the correlation between recurrence and metastasis and clinical indicators, and the possible relationship between AR and prognosis. The experimental data were statistically analyzed using SPSS 24.0 statistical software.Results:1.Among the 94 patients with TNBC,a total of 26 patients had positive AR expression, and the positive rate of AR was 27.7%.Compared with the clinical indexes of TNBC patients, the expression of AR was positively correlated with tumor size and Ki-67 expression, which was statistically significant(p<0.05),suggesting that the larger the tumor in TNBC patients, the higher the expression of Ki-67,the higher the positive rate of AR;but there was no significant correlation with age, lymph node metastasis and clinical stage(p>0.05).2.Of the 94 patients,17 had recurrence, and 7 of them had positive AR expression. The proportion of recurrence in 26 AR-positive patients was 23.0%,and the proportion of recurrence in 68 AR-negative patients was 16.1%.By comparing the recurrence and metastasis of TNBC patients with the relevant clinical indicators,it was found that the recurrence and metastasis of TNBC patients was positively correlated with tumor size, lymph node metastasis, clinical stage,Ki-67 expression, and AR expression which was statistically significant(p<0.05),suggesting that the larger the tumor size in TNBC patients, the lymph node metastasis, the higher the clinical stage, the higher the expression of Ki-67,AR expression positive, the more prone to recurrence and metastasis;there was no significant correlation with age(p>0.05).3.Survival curves were used to analyze the relationship between the three-year disease-free survival(DFS)and overall survival(OS)between AR and TNBC patients, and there was no significant correlation(p>0.05).Univariate analysis showed that lymph node metastasis and clinical stage were associated with DFS and OS in patients with TNBC.Multivariate analysis showed that AR could not be used as an independent prognostic factor for TNBC.Only lymph node metastasis can significantly predict recurrence, which is an independent effect of prognosis in patients with TNBC.Factors(p<0.05).Conclusion:1.Patients with TNBC are prone to recurrence and metastasis when AR expression is positive, and AR may be associated with poor prognosis in patients with TNBC;2.The expression ratio of AR positive in TNBC patients was higher in tumor size T3 group and Ki-67>20%group.3.AR may be a potential therapeutic target for TNBC.4.There was no significant correlation between the expression of AR and DFS and OS in patients with TNBC for 3 years.Regression analysis indicated that only lymph node metastasis could be used as an independent prognostic factor for TNBC patients.
