Correlation Between Postoperative Anemia And Clinically Relevant Delayed Gastric Emptying After Pancreaticoduodenectomy

Objective:To investigate the correlation between postoperative anaemia(PA)and clinically relevant delayed gastric emptying(CR-DGE)after pancreaticoduodenectomy(PD).Methods:A retrospective analysis of 56 patients with pancreaticoduodenectomy in the First Affiliated Hospital of Kunming Medical University from September 1,2016 to December 31,2018.(1)According to whether there was postoperative anemia,it was divided into anemia group(27cases)and non-anemia group(14 cases).The difference of CR-DGE,gastric tube extraction time and postoperative hospital stay between the two groups was studied by statistical methods.(2)According to whether there was delay in gastric emptying,it was divided into CR-DGE group(26)and non-CR-DGE group(30).Univariate analysis and logistic regression analysis were used to study the effects of anemia and other factors on clinically relevant gastric emptying delay.Results:There were 30 cases of anemia after operation,the incidence rate was 53.6%(30/56),and 26 cases of CR-DGE occurred,the incidence rate was 44.6%(26/56).(2)Compared with the non-anemia group,the incidence of CR-DGE in the anemia group was 70.37%(19/27),which was significantly higher than that of the non-anemia group,except for the currently known risk factors that may affect the gastric emptying disorder.The group was 35.71%(5/14),and the difference between the groups was statistically significant(P<0.05).After t test,the time of gastric tube removal and postoperative hospital stay in the anemia group were larger than those in the non-anemia group,the difference was statistically significant(P<0.05).(2)CR-DGE group and non-CR-DGE group,postoperative anemia(x2=7.724,P=0.006);abdominal lymph node dissection(x2=5.824,P=0.025);pancreatic duct diameter less than 0.3cm(x2=5.013 P=0.047),The differences were statistically significant..Logistic regression analysis of the above three factors suggested that postoperative anemia was an independent risk factor for DGE after PD(p<0.05).The differences were statistically significant.Conclusion(s):The occurrence of CR-DEG after PD was significantly associated with postoperative anemia,abdominal lymph node dissection and pancreatic duct diameter less than 0.3 cm.Postoperative anemia was an independent risk factor for DEG.
