| Objective(s):Cardiovascular magnetic resonance feature tracking(CMR-FT)was used to analyze the myocardial segmental strain and cardiac function in patients with left ventricular myocardial incompatibility(LVNC).Longitudinal strain(LS),Radial strain(RS),and Circumferential strain(CS)of incompletely segmented,non-densified segments and normal control groups Differences and differences in left ventricular ejection fraction,left ventricular end-diastolic volume(LVEDV),and left ventricular end-systolic volume(LVESV).Methods:During the period from January 1,2016 to August 30,2018,22 patients(LVNC group)with LVNC were diagnosed by clinical and CMR in the Second Affiliated Hospital of Kunming Medical University,and 12 healthy controls(control group)were included in the same period.All patients and the control group underwent CMR cardiac film examination.The cardiac film examination images were processed by CVI42(Circle Cardiovascular Imaging,Canada)software CMR-FT technology to obtain the cardiac function parameter values(LVEF,LVEDV,LVESV)and strain parameter values(LS,RS,CS).Each segment was analyzed and grouped using the American Heart Association(AHA)17-segment method(this group did not include Section 17):1.The myocardial segment of the ILVNC group was divided into a densification segment(group A)and a non-densification segment(group B),and the control group was a normal myocardial segment(group C).2.The LVNC group was further divided into Aa group(EF<50%)and Bb group(EF≥50%)according to LVEF,and was compared with healthy control group(Cc group).Statistical analysis was performed on each of the above groups by analysis of variance and rank sum test.Results:1.A total of 352 segments in the ILVNC group and 85 incomplete segments(24%).The average number of myocardial segments in each patient was 3.9±2.1.267 non-densified segments(76%),The average number of non-densified myocardial segments per patient was 12.1 ± 2.1.The incidence of myocardial densification in the segment was apical wall(5.40%),middle anterior wall(4.26%),and middle lower wall(3.69%).2.The LVEF value(%)of the ILVNC group was[46.26±17.69],the LVEDV value(ml)was[88.80±57.80]ml,the LVESV value(ml)was[56.38 ± 50.50]ml,and the LVEF value of the control group(%).)is[59.86±5.79],LVEDV value(ml)[63.14±20.63]ml,LVESV value(ml)[28.20±11.24]ml.There were significant differences in cardiac function parameters between the ILVNC group and the control group(P<0.05).3.The median RS of group A was 18.42%(12.26%,29.67%)the median of CS was-14.02%(-19.46%,-8.67%),LS median The number is-10.63%(-14.32%,-5.65%);The median RS of group B was 23.76%(14.40%,38.42%),the median CS was-13.87%(-18.9%,-9.32%),and the median LS was-8.03%(-15.18%,-2.43%);The median RS of group C was 29.06%(21.74%,41.56%),the median of CS was-19.27%(-22.58%,-15.49%),and the median LS was-13.78%(-19.37%,-7.86%).All indicators in group A and group C were different(P<0.05);All indicators in group B and group C were different(P<0.05);There were differences in RS parameters between group A and group B(P<0.05),and there was no difference between CS and LS parameters.4.The median RS of the Aa group was 15.56%(8.70%,27.16%),the median CS was-10.78%(-14.43%,-6.84%),LS median The number is-6.48%(-11.07%,-1.7%);The median RS of the Bb group was 30.30%(21.73%-43.66%),the median CS was-18.16%(-21.83%,-14.01%),and the median LS was-13.70%(-18.93%,-6.75%);The median RS of the Cc group was 29.06%(21.74%,41.56%),the median CS was-19.27%(-22.58%,-15.49%),and the median LS was-13.78.%(-19.37%,-7.86%).All indicators in group Aa and group Bb were different(P<0.05);All indicators in Aa group and Cc group were different(P<0.05);There were no differences in all parameters between the Bb group and the Cc group.Conclusion(s):1.CMR can clearly show the distribution of incomplete segments.The most common site of myocardial densification is the side wall,and the apical side wall is the most common.2.The cardiac function of patients with ILVNC was lower than that of the healthy control group,suggesting that the myocardial densification segment had an effect on cardiac function.3,ILVNC patients with incomplete myocardial segment,non-densified incomplete myocardial segment compared with the healthy control group showed strain reduction,the worst strain of myocardial densification segment.4.Patients with LVEF<50%of ILVNC had a myocardial segmental strain value that was lower than LVEF ≥ 50%and strain in the healthy control group,suggesting a decrease in cardiac function in patients with reduced ILVNCLVNC myocardial segmental strain. |
