Pulmonary atresia with ventricular septal defect(PA-VSD)is a relatively complex congenital heart and lung malformation.The anatomical and physiological differences between individuals are extremely large.Computed tomography angiography(CTA)is an effective imaging technique for obtaining cardiac image in patients with PA-VSD,which plays an important role in clinical practice.Based on the CTA data of 204 patients with PAVSD from Guangdong Provincial People’s Hospital,this paper combined with medical image processing methods and statistical analysis methods for retrospective analysis.For the PA-VSD patient image data,the main methods of PA-VSD blood supply,pulmonary vascular development of MAPCAs and pulmonary vascular volume index were studied.Blood supply study was performed in 72 patients with PDA and 68 patients with MAPCAs.Statistical analysis was performed using IBM SPSS Statistics 22.0.The results showed that there were significant differences in preoperative oxygen saturation between the two groups,and the postoperative oxygen saturation was significantly improved.There were differences in height,weight,pulmonary artery and pulmonary vein index,and the patients in the MAPCAs group were found to be late.The palliative treatment mortality rate was 21.1% in the PDA group and 17.8% in the radical treatment.The mortality rate of palliative treatment in the MAPCAs group was 8.7%,and 5.9% in the radical treatment.Most of the PDA group(67%)underwent radical surgery,and the majority(72%)of the MAPCAs group chose palliative surgery.Pulmonary vascular development studies of PA-VSD-MAPCAs used Mc Goon,Nakata,and PVI as indicators.The image data of healthy infants was used as a control group.89 patients and 28 healths data were selected.Image processing was used to obtain pulmonary vascular features.Basic information was obtained from clinical treatment records,and pulmonary vascular development in healthy and patient groups was compared.The results showed that the mean values of Mc Goon and Nakata in the patient group were 1.18 and 133.01,which were significantly lower than 1.98 and 246.62 in the healthy group.The PVI of the patient group was 467.59,which was significantly higher than that of the healthy group(346.77).The study of pulmonary vascular volume index was performed on 79 cases of CTA images with good imaging quality.Image processing was used to segment and calculate pulmonary vascular volume.After analyzing the pulmonary vascular volume index of type I,II and III of PA-VSD,the results showed that the segmentation method used in the paper segmented the pulmonary vascular structure well and calculated the pulmonary vascular volume index.However,the index has no significant relationship with parameters such as Mc Goon,Nakata and preoperative blood oxygen saturation.Patients with type I,II,and III also had no significant differences in their indices.Comparing the pulmonary vascular volume index of the survival and death groups,it was found that the index of the death group of type II and type III patients was significantly lower than that of the survival group.Based on the CTA image data of PA-VSD patients and the parameters in clinical treatment,the following results were obtained:(1)The gender,surgical mode and NPA of PA-VSD patients did not affect postoperative survival.(2)MAPCAs have better blood supply than PDA,and their postoperative mortality is lower.(3)Patients with MAPCAs had significantly lower Mc Goon and Nakata than the healthy group,and the PVI index was higher than that of the healthy group.(4)Preoperative oxygen saturation was positively correlated with PVI in patients with PA-VSD-MAPCAs.(5)Patients with PA-VSD II and III had a higher mortality rate of low pulmonary vascular volume index.The above results explored the characteristics of imaging and clinical,and unearthed clinically significant features.Some basic work was done for PA-VSD research,which is helpful for subsequent research.
