BackgroundCerebral small vessel disease(CSVD)refers to a group of clinical, imaging and pathological syndromes caused by many factors, such as cerebral arterioles, arterioles, capillaries, venules and venules. Most CSVD patients with occult onset have poor clinical specificity and various manifestations.Common clinical symptoms include cognitive impairment, dysphagia, motor dysfunction and so on.The study found that the incidence of dysphagia increased year by year, patients with dysphagia easily lead to a variety of complications, such as aspiration pneumonia, malnutrition, dehydration, psychological and social communication disorders, seriously affect the ability of daily life and quality of life.To improve patients’ nutrition, enteral nutrition is divided into enteral nutrition and parenteral nutrition.Enteral nutrition is divided into persistent naso-gastric tube feeding(PNG)and percutaneous endoscopic(PEG),but PNG and PEG have their own advantages and disadvantages.Finding the best nutrition way suitable for patients has a positive effect on improving nutritional status and prognosis.Intermittent oro-esophageal tube feeding(IOE)was written in 2017 in the consensus of experts on assessment and treatment of dysphagia in china.it is a feeding method for intubating while eating and removing pipes when not eating.its main characteristics are intermittent and more convenient to operate. This study compared the clinical effect of IOE and PNG in cerebral small vessel disease, and discussed the effect of patients on swallowing function and pneumonia treatment, nutritional status, daily life ability and quality of life.ObjectTo observe the effect of IOE on swallowing function and pneumonia treatment, nutritional status, daily life ability and quality of life of patients with CSVD.Methods60 patients with CSVD dysphagia were divided into experimental group and control group according to random number table method,30 cases in each group. The two groups were given the same basic drug therapy and related swallowing training;on this basis, the control group was given IOE.PNG,each experimental group The indexes before and after treatment and the improvement of swallowing function were compared between the two groups.ResultsAfter 2 weeks of treatment, the swallowing function of the two groups was improved compared before treatment, and the experimental group was better than the control group, and the difference was statistically significant(P <0.05).The nutritional index of the two groups was improved, the BMI、ALB and HB of the experimental group were significantly higher than that of the control group, and the difference between the two groups was statistically significant(P<0.05).Compared with the control group, the treatment effect of pneumonia in the experimental group was obviously improved, and the difference was statistically significant(P<0.05).and the improvement of daily life ability and quality of life in the experimental group was better than that in the control group, and the difference between the two groups was statistically significant(P<0.05).ConclusionsIOE provide nutritional support for patients with CSVD dysphagia, which can improve their nutritional status, promote the recovery of swallowing function, promote lung rehabilitation, and improve daily life ability and quality of life.
