ObjectiveBy investigating the status of hope level and stress reaction of main caregivers of patients with acute cerebral hemorrhage, the related influencing factors were analyzed, and the correlation between hope level of main caregivers and stress response of patients with acute cerebral hemorrhage was discussed,so as to provide basis for clinical nursing intervention measures.MethodsThis study is a descriptive study. In the Department of Neurosurgery of a tertiary hospital,268 patients with acute cerebral hemorrhage and 268 primary caregivers were selected from January 2018 to January 2019 who met the inclusion criteria.General information questionnaire, stress response questionnaire(SRQ)and Herth hope index scale were used to investigate.Spss21.0 software was used to input relevant data, and t-test and ANOVA were used for analysis;multiple linear regression analysis was used to analyze the influencing factors of stress response and hope level;Pearson correlation analysis method was used to analyze the correlation between stress response and hope level of main caregivers.Result1.The total score of psychological stress response of the main caregivers of acute cerebral hemorrhage was(61.09±16.37),which was in the middle and low level.Among them, the scores of each dimension of the scale from high to low were body response score(26.01±6.54),psychological response score(21.46±5.91),and social behavior score(13.62±3.92).2.The total score of hope level of main caregivers of patients with acute cerebral hemorrhage was(36.56 ± 6.92),which was in the middle and high level. The scores of each dimension from high to low were(12.55±2.2)for maintaining close relationship with others,(12.21±2.27)for taking positive actions,and(11.81±2.45)for positive attitude towards reality and future.3.The results of univariate analysis showed that: in the course of disease,GCS score, economic burden, caregiver's gender, marital status, physical health, understanding of cerebral hemorrhage treatment level, relationship with patients, convenience of residence to hospital, daily care, understanding degree of cerebral hemorrhage related knowledge, thinking that patients are The difference was statistically significant(P < 0.05).4.The results of univariate analysis on the hope level of the main caregivers of patients with acute cerebral hemorrhage showed that: the caregivers' age, their own health status, education level, understanding of cerebral hemorrhage treatment level, reimbursement of medical expenses, course of disease,GCS score, whether there is dysfunction, economic burden, understanding of cerebral hemorrhage related knowledge, whether the patient is in critical condition, and whether they need to take care of them at the same time His family members were different, the differences were statistically significant(P < 0.05).5.The linear regression analysis showed that the course of disease, the knowledge of cerebral hemorrhage, whether the patient was in critical condition, whether he needed to take care of other family members, economic burden and hope level were the main influencing factors of stress response, which could explain 40.7% of the variation The regression equation is y = 18.786 + 2.116x1 + 1.020x3-1.055x11 + 1.439x12 + 2.340x13 + 1.671x14.Among them, the course of disease, whether the patient is in critical condition, whether to take care of other family members and financial burden were positively correlated with stress response(P < 0.05).There was a negative correlation between the knowledge of cerebral hemorrhage and stress response(P < 0.05).6.Linear regression analysis showed that the main risk factors of hope level of primary caregivers in patients with acute cerebral hemorrhage were the age of caregivers ? 60 years old, economic burden and health status of caregivers(P < 0.05).There was a negative correlation between the age of caregivers and the level of hope. The older you are,the lower the level of hope.7.The Pearson correlation analysis between the total score of stress response and the total score of hope level in the main caregivers of patients with acute cerebral hemorrhage showed that there was a positive correlation between the hope level and the stress response(P < 0.05),the various dimensions of hope level were keeping close relationship with others, taking positive behavior, positive attitude of reality and future, and psychological reaction, physical reaction, physical reaction, positive attitude towards reality and future There was a positive correlation between behavioral response and the difference was statistically significant(P < 0.05).Conclusion1.The psychological stress response of the main caregivers of the patients with clinical acute cerebral hemorrhage is in the middle and high level. It is considered that the independent risk factors affecting the psychological stress response of the caregivers are life risk, course of disease > 7 days, need to take care of other family members, understand the related knowledge of cerebral hemorrhage, and have the influence of economic burden.2.The hope level of caregivers is in the middle and high level. The risk factors of hope level are age ? 60 years old, illness has a serious impact on family economy, and the health of caregivers is not healthy.3.The hope level of caregivers is the main influencing factor of stress response. The hope level of primary caregivers had a significant positive predictive effect on stress response.
