| Background Nonalcoholic fatty liver disease(NAFLD)is characterized as overloaded lipid deposition in hepatocytes with the exception of alcoholic,viral or other specific liver-impaired factors.It has become the most prevalent chronic liver disease in the world.The disease spectrum of NAFLD composes of simple fatty liver,nonalcoholic steatohepatitis,and even the cirrhosis.The morbidity and progression of NAFLD could be affected by gene-environment interaction.Coronary arterial disease is tightly associated with the risk of NAFLD.CAD could lead a large amount of deaths in NAFLD patients,and NAFLD is common considered as an independent risk factor of CAD.NAFLD and CAD share the common pathological mechanism,such as obesity and hyperlipidemia.Besides,NAFLD and CAD often co-exsist in one subject.Previous studies revealed several single nucleotide polymorphisms(SNP)which were related to both NAFLD and CAD.Musso and his partners firstly reported the relationship between TCF7L2 rs7903146 and the risk of NAFLD in 2009.They pointed out that this variant possesses an effect on hepatic lipid metabolism and dyslipidemia.Meanwhile,other researchers also discovered that TCF7L2 rs7903146 could affect the serum levels of TG,LDL and other serum lipid index.Based on these studies,TCF7L2 rs7903146 is believed to play a role in CAD.Currently,no study reveals the accurate effect of TCF7L2 rs7903146 on the risk of NAFLD,CAD,or on the serum lipid profiles in Chinese Northern Han population.Aim The aim of study is designed to investigate the association between TCF7L2 rs7903146 with the risk of NAFLD,CAD,and the risk of CAD in NAFLD patients in Han population in Qingdao,China.Method All subjects were enrolled in Qingdao Municipal Hospital.During December,2017 to December,2018,143 NAFLD patients,159 CAD patients,131 NAFLD with CAD patients and 212 healthy controls were collected.The demographic data and blood samples of all subjects are collected.The genotype and alleles of each sample were measured by the Agena Mass Array technology.All acquired data are analyzed with SPSS 23.0 in this study.The gender,age,BMI,Glu,TG,TC,LDL,HDL,ALT,AST,ALP,TBIL of each group were compared.The distribution of genotype and allele frequency in each group were also tested.Logistic regression is used to investigate the risk of diseases in the different genotype carriers.Result The clinical characteristics,BMI,Glu,TG,TC,LDL,HDL,ALT,AST,ALP,and GGT are different in every two groups.The minor allele frequency(MAF)of whole sample is 5.2%,which is in accordance with previous data of eastern Asia.The distribution of genotype or allele frequency in every two groups are not statistically different.The association of genotype of TCF7L2 rs7903146 and the risk of NAFLD,CAD or NAFLD with CAD is not statistically different by Logistic regression either.And no statistically significant difference was observed in blood lipid and liver function between the CT/TT carriers and CC carriers in the total sample.However,the CT/TT carriers had a lower BMI(P=0.027)and higher HDL(P=0.001)than CC carriers only in the healthy control group.Moreover,when BMI < 25,non-NAFLD patients(including the CAD group and control group)have a higher ratio of CT/TT carriers than that in patients with NAFLD(including NAFLD group and combined group).After the correction of gender,age and blood lipid,the difference is still significant.While no genotype difference is found when BMI ≥25.This suggests that in non-obese people,CT and TT are protective genotypes of NAFLD.Conclusion The CT/TT of TCF7L2 rs7903146 is a protective factor for the risk of NAFLD in the non-obese Han population in Qingdao,China. |
