Objective: Establish a long-term care assessment index system for the elderly in the community and home,and development a long-term care assessment tool for the elderly in the community and home.Methods: The semi-structured interview method was used to interview 12 elderly and their main caregivers in the community and home in Bengbu City to understand the subjective needs of long-term care for the elderly.Eight experts were selected by Delphi method and analytic hierarchy process to conduct two rounds of consultation and determine the index weights and consistency ratios at all levels to establish a long-term care assessment index system for the elderly in the community and home.Based on the assessment index system,an entry pool is established,and the Delphi method was used to select 4 experts to comment on the content and rationality of the assignment to form the first draft of a long-term care assessment tool for the elderly in the community and home.Convenience sampling was used to conduct a questionnaire survey on 360 elderly in the community and home in Bengbu City,through project analysis to delete entries and conduct reliability and validity tests to form a long-term care assessment tool for the elderly in the community and home.Results: The long-term care needs of the elderly in the community and home include:daily life care,medical rehabilitation care,spiritual comfort,social support,social participation,reducing the load of the main caregivers,and improving the home environment.The long-term care assessment index system for the elderly in the community and home includes: 4 first-level indicators and 12 second-level indicators.In the Delphi method,the effective questionnaire recovery rate of two rounds of expert inquiry was 100%,the expert authority coefficient was 0.94,and the Kendall coordination coefficient was 0.36 and 0.43(p <0.01),respectively.The long-term care assessment tool for the elderly in the community and home includes: 6 dimensions,56 items.Among them,t test: the t value of each item is greater than 3,and the significance test result is p <0.05;Pearson correlation coefficient: delete the C1,C2,C12,D1,D2,F6,and F7 items whose correlation coefficient is less than 0.4.Content validity: S-CVI is 1,I-CVI is 1.Exploratory factor analysis: KMO was 0.763 ~ 0.943,Bartlett’s spherical test p <0.001.Delete the entries with factor load less than 0.45(E2,E3,E5,E6)and then conduct exploratory factor analysis again.Each level of exploratory factor analysis extracts a common factor,and the cumulative variance contribution rate is56.08% to 83.68%.Reliability test: total Cronbach’s α coefficient is 0.968,and Cronbach’s α coefficient is 0.730 ~ 0.964 at all levels.Conclusion: The long-term care assessment index system for the elderly in the community and home is scientific,reasonable and reliable.The reliability and validity of the long-term care assessment tool for the elderly in the community and home have reached the standard,with good reliability and validity,and can accurately and effectively assess the long-term care needs of the elderly in the community and hone.
