Objectives: 1.This study aims to understand the status of life quality of patients with advanced and end-stage liver cancer and the main clinical symptoms and related influencing factors that affect the quality of life through questionnaire surveys;2.Analyze the current status and influencing factors of patients with advanced and end-stage liver cancer for peaceful treatment needs to provide a reference for the further development of tranquil treatment. Methods: Using convenient sampling methods,a total of 280 patients with advanced and end-stage liver cancer patients who met the inclusion and exclusion criteria were collected from June 2018 to December 2019 in the tumor wards of a top three hospital in Nanning City. After checking and eliminating invalid questionnaires,270 were finally included.example.The research adopts a questionnaire survey method. The content of the questionnaire refers to domestic and foreign literature and questionnaire data. The questionnaire has been verified by domestic researchers’ translation, localization and reliability and validity. The reliability and validity are good. The questionnaire includes the general information questionnaire for inpatients with liver cancer, the Edmonton Symptom Assessment System(ESAS),the MOS 36-item short form health survey(SF-36),and tranquility treatment Palliative Care Outcome Scale(POS).Finally,270 questionnaires were statistically analyzed using SPSS20.0 software.Based on the collected patient information, single factor analysis and multiple linear regression analysis were performed on the patient data to analyze the influencing factors of the quality of life of the patients with advanced liver cancer and the need for peaceful care.People who need tranquility care services. Results: In this study,a total of 280 questionnaires were distributed. After checking and eliminating invalid questionnaires,a total of 270 test papers were included, with an effective rate of 96.4%.Among them,193 were male and 77 were female, and the age was 65.3±13.2(years old). The results are as follows:(1)Of the research subjects,193 were male, accounting for 71.5%,and 77 were female, accounting for 28.5%.There are 16 people with religious beliefs, accounting for 6%,and 254 people without religious beliefs, accounting for 94%.The number of patients with religious beliefs is relatively small in the survey. The number of people under the age of 60 was 97,accounting for 35.9%,and the number of people ≥60 was 173,accounting for 64.1%.There were 134 patients with ascites, accounting for 49.6%.The serum concentration of alanine aminotransferase(ALT)was(71.98±16.23)μg/L,the concentration of aspartate aminotransferase(AST)was(59.11±12.83)μg/L,and alpha-fetoprotein(AFP)The serum concentration was(509.42±61.83)μg/L,and the pain score(NRS digital scoring method)was(3.76±1.54)points.(2)In the research subject’s concise health questionnaire(SF-36),patients with advanced and end-stage liver cancer scored in physiological function(56.58±15.84),overall health(42.57±16.53),and energy(39.81±19.59)The scores and emotional problems(53.92±21.64)are the worst in the four dimensions, and the highest score is the physical function(64.27±18.32).(3)In the Edmonton symptom assessment system(ESAS),according to the scoring results and the median and quartile of the patient’s ESAS scale score, patients with advanced and end-stage liver cancer have a pain score of 4(1,6),depression score 6(4,7),anxiety score 5(3,7),and sleepiness score 6(3,8)are the worst dimensions, with score 2(1,3)and itch score 2(1,4)and appetite score 3(1,4)symptoms were relatively mild.(4)For multiple linear regression results of factors affecting the quality of life of patients with advanced liver cancer, the coefficient of determination R2=0.527,the patient’s religious beliefs, marital status, economic level(family monthly income),liver cirrhosis and AFP level factors have a quality of life score Significant impact(P<0.05)(5)Current status of peaceful care quality for patients with advanced liver cancer: POS total score(22.57±7.43),psychological and quality of life dimension(19.28±5.36),quality of care dimension(3.48±1.85)Minute.(6)According to the difference in the needs of peaceful care for patients with advanced and end-stage liver cancer, the scores of the patient’s peaceful treatment passport protection outcome scale are mainly in marital status(t=0.738,P=0.008),religious belief(t=2.052,P= 0.044),source of medical expenses(F=3.909,P=0.016),AFP index(t=1.632,P=0.034),economic level(F=3.939,P=0.027),cirrhosis(t=1.951,P =0.039),HBV prevalence(t=2.931,P=0.045),ascites(t=1.247,P=0.029),and pain level(t=2.789,P=0.047)were statistically significant, and there was no difference in other items Statistical significance.(7)In the multiple linear regression of the patient’s need for tranquility and care, the determination coefficient R2=0.612,and the five independent variables included in the model(with or without liver cirrhosis,AFP level, economic level, presence and absence of ascites and pain scores)have statistics Academic significance(P<0.05).The results showed that positive diagnosis of liver cirrhosis,AFP level, economic level factors, ascites and pain scores had a significant impact on the patient’s need for peaceful care(P<0.05).Conclusion: 1.According to the single factor analysis and multiple linear regression results that affect the patient’s quality of life, the patient’s problems in physiological functions, overall health, energy and emotion are more prominent, and the patient’s religious beliefs, marital status, liver cirrhosis,AFP level,Economic level(family monthly income)has a more significant impact on the quality of life of patients. The quality of life of patients with advanced liver cancer is affected by a variety of factors, suggesting that targeted care is needed in nursing.2.The main symptoms that affect the quality of life of patients with advanced and end-stage liver cancer are pain, depression, anxiety and drowsiness, which are the symptoms that need clinical attention.Other symptoms are relatively mild.3.Patients with advanced and end-stage liver cancer have a greater need for tranquillity care.In this study, among the many indicators of patients, the five most relevant indicators were screened-whether they were liver cirrhosis,AFP level, economic level, whether ascites and Pain scores can affect patients’ need for tranquillity care, and patients with cirrhosis, high AFP levels, ascites, and high pain scores have a higher need for tranquillity care, and patients with higher economic levels have a greater need for tranquillity care low.
