Object:To analyze the incidence and prognostic factors of acute renal function injury in patients with cirrhosis ascites in our hospital according to the new diagnosis and management opinion of the International Ascites Club(International Club of Ascites,ICA)in 2015.Methods:Selected 136 patients who met the diagnostic criteria of acute renal injury(acute kidney injury,AKI)AKI promulgated in2015.Starting from the date of diagnosis of the patient,the survival status of the patient was 90 days after follow-up.Retrospective study was used to analyze the incidence of each classification of acute renal function injury in the patients enrolled,and logistic regression was used to analyze the prognostic factors associated with AKI 90 days in patients with cirrhotic ascites.Results:(1)Cirrhosis ascites patients complicated with AKI are more common in middle-aged and elderly patients,the ratio of men and women4.4:1,the most common cause of infection due to B virus,followed by alcoholic liver disease.(2)Cirrhotic ascites were associated with aki incidence of 33.50%,of which AKI stage1 was 16.5%,AKI stage2 was4.93%,AKI stage3 was 1.97% and HRS-AKI was 10.10%.(3)AKI incidence of ascites complicated by cirrhosis was 33.50%.The incidence of each type was 16.5% in the AKIⅠ stage,4.93% in the AKIⅡstage,1.97% in the AKI Ⅲ stage,and 10.10% in the acute renal injury(Acute kidney injury of hepatorenal syndrome type,HRS-AKI)with hepatorenal syndrome.(4)Univariate analysis showed that white blood cell,potassium,blood urea nitrogen,creatinine,total bilirubin,albumin,prothrombin time,international normalized ratio,hepatic encephalopathy,infection,Child grade,Meld score were associated with 90 days death of acute kidney injury in patients with cirrhotic ascites.(5)Multifactorial division revealed that white blood cell,potassium,total bilirubin,albumin,hepatic encephalopathy were important factors for the death of acute kidney injury for 90 days in patients with cirrhotic ascites.Conclusion:1.For patients with cirrhotic ascites,the incidence of acute kidney injury is 33.5%,and the higher the AKI stage,the higher the mortality rate.2.White blood cell,potassium,urea nitrogen,creatinine,total bilirubin,albumin,prothrombin time,internationalization marker value,hepatic encephalopathy,infection,Child grade,Meld score and other factors are important factors influencing the prognosis of patients with cirrhosis and AKI for 90 days.3.White blood cell,potassium,total bilirubin,albumin,hepatic encephalopathy and other factors are independent risk factors for 90 days prognosis of patients with cirrhosis and AKI.
