Objective: To observe the clinical efficacy of Taohong Siwu Decoction in preventing the formation of deep venous thrombosis(dvt)in lower limbs after internal fixation of intertrochanteric fractures,in order to further optimize the clinical treatment of dvt formation,and to explore the prevention of dvt Clinical use value.Methods: From January 2018 to January 2020,elderly patients who underwent internal fixation surgery in the Department of XX of the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine for femoral intertrochanteric fractures were selected through screening and 60 patients meeting the inclusion criteria were randomly divided into treatments.Group and control group,30 cases in each group.The treatment before admission was the same in the two groups.The control group was treated with low molecular weight heparin for anticoagulation.The treatment group was given Taohong Siwu Decoction orally on the basis of the control group.There were no differences in treatment measures.D-dimer,prothrombin time(PT),and activated fraction were observed in the two groups of patients before and 1 dayafter surgery,1 week after surgery,and 2 weeks after surgery.The thromboplastin time(APTT),VAS score,and thigh circumference of the affected limb were changed.After the study was completed,SPSS 22.0was used for statistical analysis of the data obtained from the study.Results: 1.Comparison of the general situation of gender,age,and surgical site between the two groups of patients,p values were> 0.05,and the difference was not statistically significant.2.D-dimer: There was no significant difference in D-dimer values ??between the two groups of patients included in the study before surgery and on the first day after surgery(P> 0.05);Taohong Siwu Decoction combined with low-molecular-weight heparin calcium after surgery The D-dimer values in the first week and the second week after operation were lower than those in the control group using only low-molecular-weight heparin calcium(P <0.05).3.pt and aptt: There was no significant difference in the pt and aptt time values between the two groups of patients included in the study before and after the first day of operation(p>0.05);the treatment group of Taohong Siwu Decoction combined with low molecular heparin calcium The pt and aptt values in the first week and the second week after operation were higher than those in the control group using low-molecular-weight heparin calcium alone,and the differences were statistically significant(p <0.05).4.vas pain score: there was no significant difference between the two groups of vas pain scores before and on the first day after surgery(p>0.05);the vas score data of the two groups were compared and treated between 1 week and 2 weeks after surgery.The groups were lower than the control group(p <0.05).5.Calf circumference change: There was no statistically significant difference between the two groups of patients before surgery,on the first day after surgery,and on the first week after surgery(p> 0.05);the calf circumference of the treatment group at the second week after surgery The value was lower than the control group(p <0.05).6.Thrombosis rate: There were 0 patients in the treatment group,and the incidence of thrombosis was 0%;2 patients in the control group,and the incidence of thrombosis was 6.67%;there was no significant difference in the incidence of thrombosis between the two groups(p> 0.05).7.Safety monitoring: no serious adverse reactions such as bleeding of important organs and pulmonary embolism were found after the treatment in the two groups;only 3 patients with bleeding at the surgical site were found in both groups,with no significant difference(p> 0.05).).Conclusion: It is concluded after the study that low-molecular-weight heparin combined with Taohong Siwu Decoction can effectively improve the hypercoagulable state of postoperative blood in elderly patients with intertrochanteric fractures,reduce D-dimer and prolong PT and APTTtime.In comparison of the incidence of deep vein thrombosis,the treatment group was 0% and the control group was 6.67%.Although the difference in the incidence of thrombosis was not statistically significant(P> 0.05),the incidence of the treatment group was lower than that of the control group.It improves the blood circulation of patients,promotes hematoma suction,changes in the diameter of affected limbs and VAS scores,and has achieved satisfactory results in swelling and analgesia.The combined intervention of traditional Chinese medicine has not increased the risk of medication and has certain Its safety is worthy of clinical promotion.
