Investigation Of Risk Factors Of Patients With Thyroid Nodules And Analysis Of TCM Constitution

Objective:By investigating the risk factors of thyroid nodules and the constitution distribution of traditional Chinese medicine,it provides a basis for the early prevention and treatment of thyroid nodules.Methods:According to the corresponding inclusion criteria and exclusion criteria,to select 680 people from May 2019 to October 2019 for the health check-up center of the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine as the subjects of investigation,According to the results of thyroid color Doppler ultrasound,it was divided into thyroid nodule group and non-thyroid nodule group.Collect basic data on all survey subjects,including gender,age,height,weight,tobacco and alcohol history,blood pressure,occupational nature,seafood consumption habits,fasting blood glucose(FBG),fasting insulin(FINS),blood lipids(TG、TC、HDL-C、LDL-C),uric acid(UA);at the same time to carry out a survey on the constitution type of traditional Chinese medicine.Establish a database,use SPSS22.0 software for statistical analysis,discuss the risk factors of thyroid nodules and the distribution characteristics of TCM constitution.Results:1.Single factor study of thyroid nodules:Of the 680 healthy physical examination population,266 had thyroid nodules and the total detection rate was 39.1%.The detection rate of female thyroid nodules was 47.6%,which was higher than the detection rate of male thyroid nodules by 30.1%,The difference was statistically significant(P<0.05).The average age of patients with thyroid nodules is(51.57±9.36)years,which is significantly higher than the average age of people without thyroid nodules(46.09±10.51)years;from the perspective of age segment,thyroid nodules are mainly concentrated in the middle-aged and elderly people over 45 years old,The older the age,the higher the detection rate of thyroid nodules,the difference was statistically significant(P<0.05).The detection rate of thyroid nodules in smokers was 44.9%,higher than the detection rate of thyroid nodules in non-smokers by 36.4%,the difference was statistically significant(P<0.05).The detection rate of thyroid nodules in people who regularly eat seafood is46.0%,which is higher than the detection rate of thyroid nodules in people who do not eat seafood by 29.2%,the difference wass statistically significant(P<0.05).The average BMI of patients with thyroid nodules is(24.91±2.37)kg/m~2,which is significantly higher than the average BMI of people without thyroid nodules(23.45±2.58)kg/m~2;with the increase of BMI,the detection rate of thyroid nodules rises High,the difference was statistically significant(P<0.05).FBG,FINS,HOMA-IR,TC,LDL-C are associated with the occurrence of thyroid nodules,and the difference was statistically significant(P<0.05);Alcohol consumption,occupational nature,diastolic blood pressure,systolic blood pressure,TG,HDL-C,UA there was no obvious correlation with the occurrence of thyroid nodules,and the difference was not statistically significant(P>0.05).2.Thyroid nodules binary logistic multivariate regression analysis:Taking the presence or absence of thyroid nodules as the dependent variable,the gender,age,smoking,seafood consumption habits,BMI,FBG,FINS,HOMA-IR,TC,and LDL-C were used as independent variables for binary logistic multivariate regression analysis.The results showed that women,age,smoking,seafood consumption habits,BMI,FINS were risk factors for thyroid nodules(P<0.05).3.Thyroid nodules TCM constitution distribution survey:Qi depression constitution(27%)>Phlegm dampness constitution(22%)>Qi deficiency constitution(18%)>Damp heat constitution(13%)>Yin deficiency constitution(7%)>Blood stasis constitution(6%)>Peaceful constitution(3%)>Yang deficiency constitution(3%)>Special physique(1%).Therefore,patients with thyroid nodules are predominantly of biased constitution,of which Qi depression constitution(27%)accounts for the highest proportion,followed by Phlegm dampness constitution,Qi deficiency constitution,Damp heat constitution;People with no thyroid nodules had the highest proportion of peaceful constitution(30%).The distribution of 9 kinds of TCM constitutions in patients with thyroid nodules and those without thyroid nodules was statistically significant(P<0.05).Conclusions:1.The occurrence of thyroid nodules is related to gender,age,smoking,seafood consumption habits,BMI,FBG,FINS,HOMA-IR,TC,LDL-C,among which women,age,smoking,seafood consumption habits,BMI,FINS is a risk factor for thyroid nodules.2.Qi depression constitution,Phlegm dampness constitution,Qi deficiency constitution,Damp heat constitution are common types of traditional Chinese medicine for thyroid nodules.
