Background Acute myeloid leukemia(AML)is a heterogeneous hematological malignancy characterized by clonal expansion of bone marrow cells in peripheral blood,bone marrow and / or other tissues.It is the most common form of acute leukemia in adults.And it is one of the largest leukemia deaths in the United States every year.And the median age of diagnosis was 67 years old,54% of patients were 65 years old or above at the time of diagnosis(about one third of patients were ≥75 years old at the time of diagnosis).However,the age of onset,survival rate and prognosis of AML patients in the Central Plains of China have not been statistically analyzed.In order to find out the factors that affect the prognosis of the patients in the Central Plains,this paper will further explore the urgency of the farmers in the Central Plains of China based on the clinical characteristics and survival time of AML patients in our hospital compared with TCGA data analysis The clinical characteristics and prognostic factors of patients with myeloid leukemia.Objective To explore the clinical characteristics and prognosis of acute myeloid leukemia patients in North Henan.Methods Adult patients with acute myeloid leukemia who were treated with chemotherapy alone and who had not undergone hematopoietic stem cell transplantation in our hospital from January 2016 to June 2018 were selected.The clinical characteristics and prognostic parameters were compared with the patient data in the TCGA database to explore the effects of chemotherapy alone.Results Compared with the patients in TCGA data,the median age of AML was younger in farmers with chemotherapy alone(56 years vs 51 years old,P=0.028);more male patients in our hospital,more female patients in TCGA database(P= 0.020);moreM2 and M5 patients in our hospital with FAB classification,more M1 and M4 patients in TCGA database(P=0.000);the proportion of patients with poor prognosis in our hospital is higher than that in TCGA database(46.6% vs 28.3%,P=0.035);there was no significant difference in OS between patients receiving chemotherapy alone and patients receiving chemotherapy alone(P=0.444);patients receiving chemotherapy alone had significant OS in patients receiving hematopoietic stem cell transplantation in TCGA database(P <0.001).In the univariate analysis of patients’ prognosis in our hospital,there were significant statistical differences in patient age,complete induction of initial induction,stratification of prognosis risk,and prognosis(P <0.05).Multivariate analysis found that age,complete response to initial induction,and stratification of prognostic risk were independent prognostic factors.Conclusion The clinical characteristics and prognosis of adult acute myeloid leukemia in our hospital are different from those in TCGA database of foreign countries.The patients in our hospital are relatively younger at first diagnosis,more male patients,more M5 in FAB classification,and more patients with poor prognosis.Age,first-time remission and risk stratification were independent prognostic factors.The total OS of adult AML patients treated with chemotherapy alone in our hospital is similar to that of patients treated with chemotherapy alone in TCGA database,which is significantly lower than that of patients treated with hematopoietic stem cell transplantation. |
