| Objective: To investigate the related factors of pulmonary hypertension(PH)in patients receiving long-term hemodialysis(HD).Methods: 73 patients(aged 18 to 80 years old)who received HD therapy in The Second Affiliated Hospital of Wannan Medical College between January 2016 and December 2019 were selected.According to the pulmonary artery systolic pressure,the patients were divided into two groups: pulmonary hypertension group and non pulmonary hypertension group.Taking Doppler echocardiography performed within 2 hours after hemodialysis,choose the results of Doppler echocardiography performed such as Left atrium(LA),Aorta,Left ventricularn end diastolic dimension,Left ventricularn end systolic dimension(LVIDs),Interventricular septum,Left ventricular posterior wall,Left ventricular ejection fraction(LVEF),Fractional shortening,Carotid intima-media thickness(CIMT).General medical history data such as the height,body mass,geriatric nutritional risk index(GNRI),inter dialytic weight gain(IDWG),and blood pressure were measured by professional nurses.Laboratory indicators of venous blood were obtained before hemodialysis.Laboratory testing for albumin,cholesterol,triglyceride,high-density lipoprotein,low-densitylipoprotein,uric acid,urea nitrogen,cystatin C,calcium,phosphorus were measured by ammonia iminohydrolase method.The single factor analysis and multivariate analysis was used to analyze the above data.statistical methods were used to explore the risk factors of pulmonary hypertension in MHD patientsResults: 1.Prevalent in 23/73 patients(31.5%).2.There were significant differences in IDWG,GNRI,dialysis time per month,LA,LVIDs,LVEF,CIMT,hemoglobin,albumin,uric acid,cholesterol between PH group and non-PH group(P<0.05).3.Multiple linear regression analysis GNRI,LVEF were the independent factor that affected the pulmonary artery systolic pressure of long-term hemodialysis patients.4.Logistic regression analysis revealed that LA,inter dialytic weight gain were independent risk factor of PH in HD patients(odds ratio 1.307,1.696).Conclusion : 1.PH is highly prevalent in MHD patients.2.Pulmonary artery systolic pressure is strongly associated with an enlarged left atrium and poor GNRI 3.LA,IDWG are independent risk factor of PH in patients receiving long-term hemodialysis... |
