Coal production process produces a lot of coal dust,coal mine workers are very susceptible to pneumoconiosis.People with pneumoconiosis of coal mine suffer from the rapid declineof respiratory function,and the quality of life is greatly reduced.Many coal miners who with pneumoconiosis patients die in misery.The majority of coal miners with pneumoconiosis patients are the main labor and economic pillar of the family,because of their incapacity and the rapid decline of family economic income.At the same time,the medical expenditure of pneumoconiosis is increasing rapidly,and the patient's family economy is overwhelmed.Poverty and the return to poverty of illness are the inevitable result of this kind of family.Therefore,it evaluates the quality of life and economic burden of coal mine workers with pneumoconiosis,analyzes the factors that affect the quality of life of coal mine workers and the factors that affect the economic burden of patients,and puts forward suggestions for improving the quality of life of coal mine workers with pneumoconiosis and reducing the economic burden of coal mine workers with pneumoconiosis.It is a necessary task for the government to strengthen the prevention and control of pneumoconiosis,medical treatment and social assistance.In this paper,pneumoconiosis patients of coal mine workers diagnosed by occupational disease diagnosis experts of Chongqing Municipal Centers for Disease Control and Prevention and those of coal mine workers hospitalized at Chongqing Coal Vocational Hospital and Chongqing Occupational Disease Prevention and Control Institute were selected as the research objects(439 people with quality of life and 167 economic burdens).The Chinese version of the SF-36 table was selected to evaluate the quality of life,and the days of hospitalization,the source of hospitalization expenses,the income from illness,the cost of nutrition,transportation expenses,and DALY were selected as quantitative indicators to evaluate the economic burden,and the economic burden was evaluated comprehensively.All survey data were established using EXCEL 2016 and the Chinese version of SPSS22.0statistical analysis was used.The descriptive statistics are represented by X ± S.The t-test is used for the comparison of the two groups of averages.The covariance analysis is used for the comparison of multiple groups,and the Q test is used for the comparison between two groups.Multivariate linear regression method is used to analyze the multi-factor influencing factors of quality of life and economic burden.Some suggestions were put forward to improve the quality of life of coal mine workers with pneumoconiosis and reduce the economic burden ofcoal mine workers with pneumoconiosis.The main findings are as follows:(1)The scores of coal mine workers with pneumoconiosis in each dimension of the SF-36 scale were physical function(37.69 ± 25.78),role physical(3.96 ± 15.27),body pain(47.05 ±29.68),general health(16.61 ± 18.08),and vitality(30.24 ±16.70),social function(45.93 ±24.88),role emotional(8.52 ± 22.43),mental health(40.54 ± 16.84).All eight dimensions scored lower than normal males and other pneumoconiosis patients.In the 8 dimensions of the quality of life of coal mine workers with pneumoconiosis,the decline in mental health was the smallest,with a 44.2 per cent decline,compared with a 95.0 per cent decline in physiological function.Factors that affect the quality of life of coal mine workers with pneumoconiosis include self-care,feelings of respect,sleeping ability,hobbies,pneumoconiosis staging,family harmony,complications,type of work,related treatment,education,smoking,place of work,and drinking.It is recommended to improve the quality of life of patients by strengthening the prevention of pneumoconiosis,treatment of complications,and external help and care.(2)The total per capita medical expenses for hospitalization of 167 coal mine workers with pneumoconiosis during the year were 5,1145.71 ± 33424.86 yuan,the per capita nutrition and transportation costs of patients and accompanying personnel were 7155.49 ±2314.21 yuan,and the per capita total direct economic burden was 58301.19 ± 35735.23 yuan.The total indirect economic burden per capita within one year was 275373.01 ± 191831.95 yuan.The per capita total economic burden is 3,33674.20 ± 217278.65 yuan.The total direct economic burden per patient in one year is 2.24 times the per capita disposable income in China in 2017,and 2.41 times the per capita disposable income in Chongqing in 2017.The total direct economic burden per patient within one year increased by 2.98 times compared with the total direct economic burden per capita generated by pneumoconiosis patients in Chongqing in 2007.The indirect economic burden was 4.41 times higher than in 2007.The total economic burden per patient in one year is 12.85 times the per capita disposable income in China in 2017,and 13.82 times the per capita disposable income in Chongqing in 2017.The factors that affect the direct medical expenses are the number of days of hospitalization and the disability level.The per capita DALY value of 167 coal mine workers with pneumoconiosis was 9.20 ± 3.04 years,and the factors that affect DALY are the disability level,pneumoconiosis stage,and disability time.It is suggested to reduce the economic burden of pneumoconiosis patients in coal mine workers by strengthening the medical facilities for pneumoconiosis treatment and the corresponding subsidies given by the government and the society.
