| Objective:Retrospective analysis was performed on the real clinical data of MDS-RAEB patients treated with DIAG regimen.It is in order to observe the therapeutic clinical efficacy,and to compare the influence of some laboratory parameters on the efficacy and prognosis.methods:A retrospective analysis of the clinical data of 60 patients diagnosed with MDS-RAEB who were admitted to the Department of Hematology of Northern Jiangsu People’s Hospital from January 1,2014 to August 31,2019.These patients were first to the hospital so their clinical data were complete.They were treated by DIAG therapeutic regimen at least 2 cycles(including 2 cycles).And all patients were diagnosed according to the WHO(2008)classfication,and stratificated according to the revised international prognostic score system for myelodysplastic syndromes.The research devote to analyze the influence of some laboratory parameters on the therapeutic clinical efficacy and prognosis.SPSS 19.0 statistical software was used for statistical analysis.T-test,Logistic regression analysis,and K-M survival analysis were used to compare the value of different factors in judging the efficacy and prognosis of such patients.P<0.05 was used to define statistically significant differences.Results:60 patients are divided into two groups.The first one is effective group,having 42 patients(70%)after the first treatment cycle,including 20(33.3%)complete remission(CR),7(11.7%)partial response(PR),6(10%)complete bone marrow remission(mCR),9(15%)hematological improvement(HI),0 stable diease(SD).Other one is ineffective group which has 18(30%)patients.Univariate analysis showed that corpuscular volume(MCV)and absolute lymphocyte count(ALC)in the effective group were significantly higher than those in the ineffective group,while lactate dehydrogenase(LDH)and serum ferritin(SF)were significantly lower than those in the ineffective group(P<0.05).Multivariate analysis suggested that mean MCV、ALC and SF are independent influencing factors.Then performing ROC curve analysis calculate the critical values and listing some factors that may affect the prognosis from documents.In the end,study reveals patients who were The number of blast of bone marrow ≤9%、age≤65、ALC≥1.15×109/L、SF≤550 ng/ml and LDH≤300 U/L have long overall survival(OS).Log-rank univariate test shows blast of bone marrow≤9%,age,LDH,and SF values were independent prognostic risk factors for these patients.Conclusions:The blast of bone marrow,age at initial diagnosis,LDH,and SF value of patients with MDS-RAEB have obvious predictive value for the prognosis of patients with DIAG therapeutic regimen. |
