Clinical Study On The Decline Of Ovarian Reserve Function Due To Kidney Deficiency By Combined Acupuncture And Traditional Chinese Medicine

Objective : To further evaluate the clinical effect of Professor Zhang Shufen’s self-made prescription combined with acupuncture in the treatment of hypofunction of ovarian reserve of kidney yin deficiency type,so that it can better guide the clinical application of DOR,and can be promoted.Methods:Based on the criteria for inclusion,,70 patients from December 2018 to January 2020 in the outpatient department of Gynecology of Shanxi Traditional Chinese medicine hospital were collected.According to the method of random and parallel control experiment,it was divided into three groups:group A(Fenodong,n=23),group B(Fenodong + Zishen Kangshuai Formula,n=23),group C(Fenodong + Zishen Kangshuai Formula + acupuncture,n=24).Continuous treatment of three menstrual cycles,the comparison of three groups of patients age,course of disease,basical Follicle-stimulating hormone(bFSH),basical Follicle-stimulating hormone(bFSH)/basical luteinizing hormone(bLH),basical Estradiol(bE2),Anti-Mullerian hormone(AMH),bilateral ovarian volume,number of follicle in basal ovary(basical antral follicle count,bAFC),and Changes of various indicators before and after treatment.And the efficacy of the three groups was evaluated 3 months after treatment.SPSS23.0 software was used for data analysis.Results:In group A,23 cases were completed and 0 cases were lost.In group B,23 cases were completed and 0 cases were lost.In group C,24 cases were completed and 1cases were lost.1.Comparison in the group:(1)In the basic serum sex hormone level:After treatment for 3 menstrual cycles,FSH levels(P<0.05)、 FSH/LH ratio(P <0.01)were reduced and E2 level was Increased to different degrees in all three groups,but there was no obvious effect on improving AMH levels(P >0.05).(2)The value of number of TCM symptoms:After treatment,the value of number of TCM symptoms among the groups was obviously descender than that before treatment(all P < 0.01),and the clinical symptoms improved significantly.(3)Bilateral ovarian volume and antral follicle count :For patients with decreased ovarian reserve function,the clinical efficacy of group C was more significant in promoting the increase of ovarian volume and the increase of sinus follicle count(all P < 0.01).Only partial improvement was observed in group A and group B(P<0.05).2.Comparison between groups after treatment:(1)In the basic serum sex hormone level:After 3 months of treatment,compared with group A,group C had more obvious clinical efficacy in reducing FSH levels(P<0.05)and FSH/LH ratio(P<0.05)on treatment DOR,and there was no significant difference in the residual serum sex hormone observation index between the three groups(all P = >0.05).(2)The value of number of TCM symptoms:There was A significant difference in the value of number of TCM symptoms among groups(P<0.01).Compared with group A,group B and group C had significant clinical efficacy in improving TCM symptoms,and group C had the best improvement in clinical symptoms.(3)Bilateral ovarian volume and antral follicle count:After treatment,the clinical efficacy in improving ovarian volume between the three groups was similar(all P = >0.05),but group C could effectively improve sinus follicular count(P < 0.05),compared with the other two groups.(4)The comprehensive curative effect:It was found that there was a significant difference in the treatment effect between the three groups(P<0.01).After pairwise comparison,the treatment effect in group C was the best,and the total effective rate of the treatment effect in group C was 74%.Conclusion:1.The clinical effect of Zishen Kangshuai prescription combined with acupuncture in the treatment of ovarian reserve function decrease is definite.It can increase the value of number of TCM symptoms(menstrual volume,waist,knee acid,soft and menstrual cycle,Tinnitus heart palpitations,anxiety and irritability,insomnia and dreamful sleep,hot and hot sweating,etc.),FSH levels,FSH/LH,E2,bilateral antral follicles were effective.2.Zishen Kangshuai Formula combined with acupuncture in TCM symptom integral(menstrual volume,waist and knee acid and soft,Tinnitus heart palpitations,anxiety and irritability,insomnia and dreamful sleep,hot and hot sweating),FSH levels,FSH/LH,antral follicles was better than that of Fenodong.3.The clinical research process of zishenkangshuai formula combined with acupuncture and the follow-up after treatment showed no obvious adverse reactions and drug side effects.It can be considered that this method is safe and reliable for the treatment of patients with ovarian reserve function decline(kidney yin deficiency type),with clear clinical effect,which is worth widely used in clinical practice.
