An Analysis Of The Effect Of Primary Total Knee Arthroplasty In The Treatment Of Valgus Knee With Patellar Dislocation

Objective: to explore the clinical effect of primary Total Knee Arthroplasty(TKA)for the treatment of valgus knee deformity with aged patellar dislocation.Methods: the clinical data of 27 patients(36 knees in total)with valgus knee combined with patellar dislocation in our hospital from January 2008 to December 2018 were retrospectively analyzed.There were 9 males and 18 females,with an average age of 67.5 years(range55-77 years).The median skin incision was made,and the medial approach was selected.In order to treat valgus deformity,the lateral collateral ligament and popliteal tendon were released successively to the lateral epicondyle of femur by the technique of gradual release of the lateral support band from the inside out and combined with the release of the iliotibial band.Bone cement prosthesis was used in all patients.Follow-up was conducted at 1 month,3 months and 6 months after surgery,and Knee lateral radiograph,full-length radiograph of both lower limbs and axial radiograph of patella were improved.Knee Society Score(KSS)and Forgotten Joint Score(FJS)were used.Paired sample t test was used for comparison before and after surgery.Results: all 27 patients(36 knees)were followed up with a mean follow-up of 4.0 years(1-7 years),and the KSS score of the affected knees increased from an average of 21 points(20-22 points)before surgery to an average of 86 points(80-92 points)after surgery,with a statistically significant difference(t=-15.155,P<0.05).Suffer from knee surgery valgus Angle by an average of 15 °(10 ° ~20 °)correction to the average postoperative 3 °(0 °-7 °),the gap was statistically significant(t = 10.388,P < 0.05),the risk of knee joint activity by preoperative an average of 80 °,60 °,100 °)better to postoperative mean 97.5 °,85 °,110 °),the gap was statistically significant(t = 14.237,P < 0.05),FJS score averaged 62.5 points(50-75),no 1case in blood vessels,nerve damage,and incision related complications,None requiredrevision surgery.Conclusion: the first total knee arthroplasty in the treatment of genu valgus deformity combined with old patella dislocation can significantly correct the knee valgus deformity and patella dislocation,improve the knee joint function,improve the life of the patients.
