| Objective:To explore the insulin requirement profiles and analyze the related factors of type 2 diabetics with different c-peptide levels on insulin pump therapy. Methods:Research object selection in December 2016 to December 2018 clinic of our hospital endocrinology,271 cases of patients with type 2 diabetes, the insulin pump intensive therapy during hospitalization, and summarizes the characteristics of insulin dosage, collect patients’general information, according to C2h/C0<2.5 ng/ml divided into c-peptide ratio is low,C2h/C0≥2.5 ng/ml for c-peptide ratio high group, and the quantity analysis before a meal quantity proportion of the differences and explore the related factors may affect the insulin dose. Results:The average insulin dose per kilogram(TDD)was(0.54±0.14)U/kg,and the proportion of basic insulin dose in patients with low C-peptide level was higher than that in patients with normal C-peptide level(P<0.05).Pearson’s correlation analysis demonstrated that there was a correlation between the ratio of total basal insulin dose(TBD)to TDD(%TBa)and body mass index(BMI)(r=0.136,P<0.05).There was a correlation between C2h/C0 and waist circumference(WC),glycosylated hemoglobin A1c(HbA1c),FBG and 2 h-postprandial blood glucose(2hBG)(r=-0.137,-0.154,-0.471,-0.172,all P<0.05).Multiple linear regression analysis showed that BMI and FBG were independent factors of%TBa(β’=0.124,0.144,all P<0.05),BMI and FBG were independent factors of C2h/C0(β’=-0.134,-0.502,all P<0.05).Conclusion:The basal premeal dose ratio of type 2 diabetes mellitus with different C peptide levels was different when intensive insulin pump therapy. The ratio of total basal insulin dose to total bolus insulin dose is 50%:50%in Chinese type 2 diabetics with low C peptide level and that of patients with normal C peptide level was close to45%:55%.%TBa was related to FBG andβcell function and BMI.The base amount of insulin pump can be set according to the function of islet. |
