| ObjectiveThis study conducted a randomized clinical trial to investigate the effect of modified fire dragon moxibustion therapy on the degree of pain,lumbar dysfunction and the efficacy of TCM syndromes in patients with kidney yang deficiency lumbar disc herniation,and under the guidance of meridian balance theory to observe the changes of meridian measurement values of some Du Meridian points before and after treatment of patients with kidney yang deficiency type lumbar disc herniation,aiming to explore the clinical efficacy of improved fire dragon moxibustion therapy,and t o provi de objective TCM evidence of evaluation of improved fire dragon moxibustion for treating kidney yang deficiency lumbar disc herniation.MethodA randomized,unblinded controlled study was performed in 66 eligible patients with lumbar disc herniation and low back pain.They were randomly divided into control group(rough salt hot pack external appl icat ion+routine care)and experimental group(improved fire dragon moxibustion therapy+routine)Nursing)33 cases each.The VAS pain visual scoring method,the lumbar spine function JOA score(hereinafrer referred to as JOA),the traditional Chinese medicine kidney yang deficiency syndrome symptom score,and the acupoint meridian measurement were used to evaluate the curative effect.The study course kept 20 days,a total of 5 treatments,and the efficacy evaluation was performed before and after each treatment.Result1.A total of 66 subjects were enrolled in the study and randomly divided into 2 groups,including 33 experimental groups,6 males and 27 females with an average age of 48.52±13.62 years.The control group consisted of 33 people,including 4 males and 29 females,and the average age was 43.48±13.15 years old.2.Repeated measures analysis of pain score:the pain scores showed an improvement trend with the increase of the number of treatments,and the difference in pain scores between the two groups was statistically significant(P<0.05),so it could be considered that the improved fire dragon moxibustion and enthusiasm both hot and cold ironing could decreased the patient’s pain,and the effect of the improved fire dragon moxibustion was better than the control group.3.The score of JOA low back pain symptom:after the treatment,the JOA low back pain symptom score in the experimental group was not significantly different from that in the control group(P≥0.05),but the overall score increased compared with before treatment,wich indicated that both could improve the patient’s low back pain dysfunction,but the difference in efficacy between the two was not statistically significant(P≥0.05).4.Quantification scores of syndromes of kidney-yang deficiency syndrome in traditional Chinese medicine:after the treatment,the scores of kidney yang deficiency syndrome in the experimental group were statistically significant compared with the control group(P<0.05),and the overall scores of the two groups were lower than before the treatment.Both of them improved the symptoms of kidney yang deficiency syndrome in patients,and the experimental group has better curative effect.5.The analysis of variance of the meridian value of Du Meridian points was carried out.The interaction between the two groups of Dumaimeng and Dazhui points was statistically significant(P<0.05).Increasing the number of treatments can increase the meridian value of Dumaimeng and Dazhui points;the factors between the Mingmen points are statistically significant(P<0.05),which suggested that the meridian values of the two groups were different,it suggesting that the fire dragon moxibustion therapy improved the Meridian value of Mingmen points better than the control group.However,there was no significant difference between the Dazhui group(P≥0.05),which suggesting that the trend of the two groups was consistent.It did not show that the improved fire dragon moxibustion had more advantages than the coarse salt hot bag.The meridian value of the Houxi point(right)of the two groups was statistically significant(P<0.05),and the other observation time points were analyzed,or the interaction with the group and the inter-group factors were not statistical.The significance of learning(P≥0.05)suggests that with the increase of the number of treatments,only the meridian value of Houxi(right)is improved.The trend of meridian values of Houxi(left)and Houxi(right)is consistent.It was suggested that the meridian values of Houxi acupoints in the two groups had no relation to the treatment.Conclusion1.Improved fire dragon moxibustion therapy and crude salt hot pack therapy could improve the pain of patients with lumbar disc herniation,and the effect of improving fire dragon moxibustion therapy was better,and it was worthy of clinical promotion.2.Improved fire dragon moxibustion therapy and crude salt hot pack therapy could improve the lumbar function score of patients with lumbar disc herniation,but combined with the study of pain score,it was considered that the main improvement was still to improve the pain,but to improve the lumbar disc.In terms of dysfunction,the two still lack strong evidence to support,suggest ing that whether it is improved fire dragon moxibustion therapy or crude salt hot shoulder bag therapy,it is currently mainly to improve the symptoms of pain in patients with lumbar disc herniation.3.For the symptoms of kidney yang deficiency syndrome in traditional Chinese medicine,improved fi re dragon moxibustion therapy and crude salt hot sputum therapy can be improved,but the improved fire dragon moxibustion therapy is obviously better than the coarse salt hot sputum therapy,which fully reflects the improved fire dragon moxibustion.The therapy was more representative of the characteristics of traditional Chinese medicine for syndrome di fferentiation.4.Improved fire dragon moxibustion and coarse salt hot wrap have certain effects on the Mingmen point of the direct action site and the meridian elevation of the Dazhui point,which were basically consistent with the pain score and the syndrome score of the kidney yang deficiency syndrome,but for the non-direct action site.The improvement of acupoints was not clear,and it was considered that both therapies mainly act on local acupoints.Secondly,it was better to improve the meridian value of the fire dragon moxibustion in the Mingmen point than the coarse salt hot pack,which suggested that the improved fire dragon moxibustion has more targeted effects on the subjects with kidney yang deficiency type. |
