Objective:To observe the clinical efficacy of radix aucklandiae tea combined with tiopronin in the treatment of mild and moderate alcohol fatty liver disease.And through cell experiments,to explore the specific mechanism of costunolide,the effective component of the traditional Chinese medicine radix aucklandiae,in the treatment of alcoholic fatty liver.Methods:(1)Clinical study:72 patients diagnosed with mild to moderate AFLD meeting the inclusion criteria of this topic were randomly divided into a treatment group(conve ntional western medicine plus radix aucklandiae tea)and a control group(only conventio nal western medicine)There are 36 patients in each group,and the total course of treat ment in both groups is 8 weeks.Observe and record the changes in liver function,blood lipids,color Doppler ultrasound and other indicators before and after treatment,and anal yze the efficacy.(2)Mechanism study:establishing AFLD cell model.MTT method to det ect the effect of lignans on hepatocyte viability;to detect the effects of linolide to alanin e aminotransferase and aspartate aminotransferase in liver cells;oil red O staining,total cholesterol and triglyceride kit to detect The effects of hydrocarbons on hepatocyte lipids;PCR and WB methods were used to detect the effects of lignans on the expression of 1 ipid regulatory factors and adenylate-activated protein kinases.Results:(1)(1)Clinical studies:1.Liver function indicators:After treatment,the control group(only conventional western medicine treatment)and the treatment group(conventi onal western medicine treatment plus radix aucklandiae bubble tea)had improved liver fu nction indicators compared with before,P<0.01,statistically significant.Comparing betwe en the groups,there was not much difference in efficacy between the control group(onl y conventional western medicine treatment)and the treatment group(conventional western medicine treatment plus radix aucklandiae bubble tea)in improving liver function.P>0.05,not statistically significant 2.Blood lipid index:After treatment,The blood lipid indi ces were prominently lower(P<0.01).In terms of reducing TG,the treatment group(con ventional western medicine treatment plus radix aucklandiae bubble tea)was significantly different from the control group(only conventional western medicine treatment)(P<0.05).The treatment group(conventional western medicine treatment plus radix aucklandiae bubble tea)was superior In the control group(only conventional western medicine treatm ent);in terms of reducing TC,the treatment group(conventional western medicine treatm ent plus radix aucklandiae bubble tea)was statistically different from the control group(only conventional western medicine treatment)(P<0.05).This shows that the combined t reatment of AFLD with traditional Chinese medicine and Western medicine is better than using western medicine alone in improving blood lipids.3.Color Doppler ultrasound ind ex:The effective rate of the treatment group(conventional western medicine plus radix a ucklandiae bubble tea)was 75.00%,and the effective rate of the control group(only co nventional western medicine treatment)was 63.89%,P>0.05,which was not statistically s ignificant.4.Comprehensive effect:In terms of effectiveness,the treatment group(conve ntional western medicine plus radix aucklandiae bubble tea)was 80.56%,and the control group(only conventional western medicine treatment)was 75.00%,P>0.05,with no sta tistical difference.(2)Mechanism study:costunolide can inhibit ethanol-induced hepatocyt e damage,and it relies on activating AMPK to regulate the expression of lipid regulatio n-related transcription factors in hepatocytes.Conclusion:radix aucklandiae tea combined with tiopronin does a great help to the treat ment of mild and moderate AFLD,which can be beneficial to the patients’symptom,pe rfectly ameliorate the TC and TG indexes,and have no obvious toxic and side effects.c ostunolide,The effective component of radix aucklandiae,through activating AMPK,the n regulates the expression of lipid metabolism-related transcription factors SREBP-lc and PPAR a,and improves ethanol-induced liver cell damage and steatosis.
