| ObjectiveTo investigate the prevalence of carotid artery plaque in with type 2 diabetes,analyze the factors which are related to carotid artery plaque and the severity of the carotid artery plaque in the elder with diabetes mellitus(DM),and then provide scientific basis for the corresponding clinical practice for DM and health policy formulation for diabetic patients.Methods1.469 cases of elderly patients with type 2 diabetes mellitus were selected and their clinical characteristics were collected from endocrinology department of the Affiliated Hospital of The Tai Shan Medical University.Thesis patients were divided into the plaque group and the non-plaque group according to the ultrasonic diagnostic results.2.Logistic regression to analyze the factors which are related to carotid artery plaque for the elder patients with type 2 diabetes mellitus.3.According to the carotid artery plaque integral semi-quantitative method,analysis the risk factors for the severity of carotid plaques of elderly patients with type 2 diabetes complicated with carotid plaques.Results1.There were statistically significant differences in the body mass index(BMI),systolic blood pressure(SBP),triglycerides(TG),high density lipoproten-cholesterol(HDL-C),apolipoprotein AI(Apo AI),Apo B/Apo AI,homocysteine(Hcy)and thyroid stimulating hormone(TSH)between the plaque group and the non-plaque group(p<0.05).2.The logistic analysis showed that the body mass index(BMI),Apo B/Apo AI,homocysteine(Hcy)and thyroid stimulating hormone(TSH)were the independent risk factors for the occurrence of carotid atherosclerosis plaque in elderly patients with type 2 diabetes.3.The logistic analysis showed that Apo B/Apo AI and homocysteine(Hcy)were the independent risk factors for the severity of carotid atherosclerosis plaque in elderly patients with type 2 diabetes.4.There were statistically significant differences in the carotid artery plaque integral between Apo B/Apo AI greater than or less than 0.88(p<0.05),and the carotid artery plaque integral of the group of Apo B/Apo AI<0.88 were lower than the group of Apo B/Apo AI≥0.88.5.The ROC curve revealed that the optimal Hcy cut off value for detecting the severity of carotid atherosclerosis plaque was 12.35umol/L,which had a sensitivity of 71.2% and a specificity of45.5%(95%CI:9.58~15.72,p<0.001).Conclusion1.Such as obesity,hypertension,hyperlipoidemia,homocysteine and thyroid stimulating hormone abnormalities were play a certain role in the development of carotid artery plaque in elderly patients with type 2 diabetes mellitus.2.The body mass index(BMI),Apo B/Apo AI,homocysteine(Hcy)and thyroid stimulating hormone(TSH)were the independent risk factors for the occurrence of carotid atherosclerosis plaque in elderly patients with type 2 diabetes.3.Apo B/Apo AI and homocysteine(Hcy)were the independent risk factors for the severity of carotid atherosclerosis plaque in elderly patients with type 2 diabetes. |
