| Objective:To analyze the lumbar paravertebral muscles(the multifidus muscles at the upper edge of the L3 and L4 vertebral bodies)by functional magnetic resonance imaging(Blood Oxygen Level Imaging BOLD,T2-mapping)imaging of the lumbar paraspinal muscles in normal healthy volunteers.Relationship between exercise intensity of the longest muscle and iliac rib muscle and its corresponding cross-sectional area(CSA),transverse relaxation time(T2 value)and effective transverse relaxation rate(R2*value),to explore the lumbar paravertebral.The trend and characteristics of signal changes in muscle magnetic resonance imaging provide a reliable theoretical guidance for clinical lumbar back muscle rehabilitation training and exercise training.Methods:120 healthy adult volunteers are selected in this study,and the selection criteria are strictly controlled.The basic physical condition of each group is about the same(P>0.05),including 60 males and 60 females,aged 19-30 years.The average age(24.3 ± 1.9)years old,according to different exercise intensity,the volunteers are divided into 4 groups,each group of 30(15 males and 15 females).In this study,the movement of the lumbar paravertebral muscles is performed on the simple Roman stool,and the upper flexion and extension exercises are performed on the simple Roman stool.The 15 flexion and extension movements are performed as a group of exercises,and are grouped according to the number of exercise groups completed by the volunteers,respectively,for the 0 group of exercises(before exercise).1 group of sports,3 groups of sports,5 groups of sports,the whole movement process has a special person to guide posture and counting.Using the PHILIPS Achieva 3.0T Tx magnetic resonance scanner,four groups of healthy volunteers with different exercise intensity are scanned for the T2WI sagittal position,the T1WI transverse position,and the T2-mapping and BOLD cross-sectional scans on the L3 and L4 levels.The images are obtained and sent to GE’s ADW4.4 image workstation for post-processing analysis.The contours of the lumbar paravertebral muscles(multi-fissure,longissimus,iliac ribs)are outlined on the upper edge of the L3 and L4 vertebral bodies,respectively.The CSA,T2 value,and R2*value are simultaneously reconstructed from the pseudo-color map of T2-mapping and BOLD.Data are analyzed using SPSS 25.0 statistical software.Results:There are different degrees of correlation between CSA,T2 and R2*values of the lumbar paravertebral muscles and exercise intensity.After the exercise intensity,the total CSA value of the lumbar paravertebral muscles increased compared with that before exercise.However,among the groups with different exercise intensity,only the total CSA of the lumbar paravertebral muscles increase significantly after exercise in 5 groups.Significance(P=0.015),CSA after exercise in 5 groups is increased after exercise in 3 groups and after exercise in group 3,but there is no statistical difference after exercise.At different vertebral levels,the total CSA of the lumbar paravertebral muscles at the upper margin of L4 and L3 vertebral bodies is significantly increased after exercise in group 3 and after exercise in group 5,and the CSA after exercise in group 1 of L4 vertebral body is higher.The movement of CSA in group 0 and group 5 after exercise is significantly higher than that in group 1(P=0.04/0.014).The total T2 values of the 3 groups of lumbar paravertebral muscles after exercise and 5 groups of exercise are higher than those of the 0 group,and the total T2 values of the 5 groups after exercise are higher than those of the 1 group(P<0.05).).At different levels,the total T2 values of the lumbar paravertebral muscles in the upper limbic layer of the L4 vertebral body,the postoperative movements of the three groups,and the movement of the five groups are higher than those of the group 0,while the lumbar paravertebral muscles of the upper margin of the L3 vertebral body are 3 After exercise,the total T2 values of the 5 groups after exercise are higher than those of the 0 group and 1 group after exercise,and the difference is statistically significant(P<0.05).The total R2*values of the lumbar paraspinal muscles after exercise,the 3 groups after exercise,and the 5 groups after exercise are lower than those of the 0 group,while the total R2*values of the 3 groups after exercise and 5 groups after exercise are lower than those of the 1 group after exercise.The difference is statistically significant(P<0.05).At different levels,the total R2*values of the lumbar paravertebral muscles in the upper limbic layer of the L4 and L3 vertebral bodies are lower than those in the 0 group after exercise,and after 3 groups of exercise,and the lower limbs of the L4 vertebral body are at the lumbar vertebrae.After the exercise,the total R2*value of the muscles of the 3 groups is lower than that of the 1 group,and the total R2*value of the 5 groups of the lumbar paravertebral muscles on the upper edge of the L3 vertebral body is lower than that of the 1 group,and the difference is statistically significant(P<0.05)Conclusion(s):The CSA and T2 values and R2*values in the functional magnetic resonance sequence T2-mapping and BOLDMRI are associated with low/moderate exercise intensity,while R2*and T2 values are more closely related to exercise intensity.It is feasible to evaluate the physiological state of lumbar paravertebral muscles after different exercise intensity by BOLD and T2-mapping.It can be used to evaluate the shape of muscles,the water molecules in muscle gap and the changes of local blood flow micro-perfusion after different exercise intensity.Clinical rehabilitation training and physical training select the appropriate exercise intensity.In this study,with the increase of exercise intensity,R2*value decreased rapidly to a level,if increased exercise intensity,R2*value would decrease gently and even maintain plateau.Total T2 value increased with exercise intensity.Total CSA increased with the increase of exercise intensity,but gradually slowed down. |
