Objective:Oral lichen planus is a disease with high incidence and difficult to cure among oral mucosal diseases.Gingival lesions are relatively more difficult to heal.Because of its site characteristics,poor drug effect,possible systemic contraindications and aesthetic effects,it is of great practical value to seek minimally invasive and effective treatment methods.In this paper,the short-term efficacy of low-energy laser and triamcinolone acetonide ointment was compared to treat gingival lichen planus,explore and evaluate the clinical application advantages of low-energy laser treatment,thus directing clinical practice.Methods:According to the inclusion and exclusion criteria,90 patients diagnosed as erosive gingival lichen planus in the emergency department and mucous membrane department of the stomatological hospital affiliated to China medical university were selected and randomly divided into 3 groups,with 30 patients in each group.Patients in each group were gargled with sodium bicarbonate for one minute before medication or laser treatment.Group A: Triamcinolone acetonide ointment is applied topically once a day and before sleep for one month.Group B: local irradiation with low-energy laser once per two days for 15 times;Group C: triamcinolone acetonide ointment and low-energy laser are combined for treatment.Low-energyr laser is irradiated for 15 times once per two days,and then the ointment is applied before going to bed,once a day,with a treatment course of one month.The three groups of patients collected relevant clinical data before treatment,including clinical indicators that may affect the disease.Observe and compare the lesion area,pain degree,symptoms and signs of each group of patients before treatment,1 week,2 weeks and 4 weeks after treatment.At the end of the 4th week,the effective rate of each group was observed.Patients who had been cured for one month were observed for their recurrence rate at 2 months.Results: After treatment,the erosive lesion area of ABC group 3 was reduced,the pain degree and symptom evaluation and physical sign scores decreased overall,and the overall effect of group C was the best.The reduction of lesion area and symptom and sign scores in group A are better than those in group B;However,the pain degree in group B decreased better than that in group C,with statistical difference between the two (p < 0.05).The highest effective rate of group C was 90% when treated for one month,while the effective rate of group B(56.7%)was lower than that of group A(83.3%)and group C.After 2 months of follow-up,group A had the highest recurrence rate(40%),which was higher than group B(23.5%)and group C(18.5%).Conclusion: The overall effect of low-energy laser combined with triamcinolone acetonide ointment in treatmenting of erosive gingival lichen planus is better than that of single drug and single laser.The speed of promoting healing of lesions by using drug alone was higher than that by using laser alone in a short period of time.However,the ability to relieve pain with laser alone is higher than that with drug alone,and the long-term recurrence rate is lower than that with drug alone.
