An Investigation Of The Opportunity Of Laparoscopic Cholecystectomy For Acute Calculous Cholecystitis

Objective To investigate the clinical effectiveness of Laparoscopic Cholecystectomy in different operative opportunity for patients with Acute Calculous Cholecystitis.Methods The clinical data of 432 patients with Acute Calculous Cholecystitis underwent Laparoscopic Cholecystectomy in The Fourth Affiliated Hospital of China Medical University from Dec.2015 to Dec.2017 were retrospectively analyzed.The patients were divided into three groups according to the time of the onset of symptoms to underwent operation.The patients in group A were performed operation with the onset of symptoms within 72 hours and 299 patients were included.The patients in group B were performed operation with the onset of symptoms between 72 hours to one week and 87 patients were included.The patients in group C were performed operation with the onset of symptoms beyond one week and 46 patients were included.The relativity between different operative opportunity and operative time,volume of blood loss in operation,rate of conversion to laparotomy,postoperative drainage time,postoperative exhaust time,postoperative hospitalization time and total postoperative complications rate among patients in the three groups were investigated and analyzed by statistical methods.Results There was no significant difference between group A and group B in operative time,rate of conversion to laparotomy,postoperative drainage time,postoperative exhaust time,postoperative hospitalization time and total postoperative complications rate(P>0.05)except the volume of blood loss in operation(P < 0.05).While for patients in group C,it was higher than those in group A and group B,and the difference was statistically significant(P < 0.05)except severe complication(P>0.05).Conclusion It’s safe and feasible for patients with Acute Calculus Cholecystitis to be performed the operation with the onset of symptoms within one week based on grasping the indications of surgery strictly.While for patients with the onset of symptoms beyond one week,specific therapeutic measures should be determined according to the patient’s overall conditions.
