| Objective:To understand and analyse the current application of prophylactic antibiotic antibiotics in acute pancreatitis(MSAP and SAP)patients in EICU of Ruijin hospital.Methods:Retrospectively analyse the MSAP and SAP patients admitted to Ruijin Hospital EICU ward from January 2012 to October 2016,record the specific application of prophylactic antibiotics in detail,summarize the risk factors that antibiotics needed to be upgraded.Results:57 patients with MSAP were selected,100% for prophylactic use of antibiotics,10 patients used cephalosporins plus metronidazole,26 patients used carbapenems,15 patients due to the initial use of antibiotics against infection wass poor,needed to be upgraded from cephalosporins to carbapenems.Through the analysis of risk factors,the etiology of the differences,and the higher the proportion of the bile group of the upgrade group,the first day APACHEII score that there is a statistical difference.There were 56 patients with SAP who were eligible for admission,and 100% for prophylactic use of antibiotics.Of these,5 patients used cephalosporins and 50 patients were treated with carbapenems.Of the 31 patients,Antibiotic antiinfection effect is poor,the need for late use of anti-positive bacteria drugs.Through the analysis of risk factors,the first day APACHEII score there is a statistical difference。Conclusion:In MSAP patients,biliary pancreatitis may require early use of higher levels of antibiotics;In MSAP and SAP patients,the severity of the disease is an important factor affecting the upgrading of antibiotics. |
