Study On Content Construction Of Health Management System For Old People At Home

Objective:To construct the content of the health management system for the old people at home,which is suitable for the national conditions of our country and meets the needs of the old people’s health management,and to provide the theoretical basis for the information construction of the health management system for the old people at home.Methods:1.Literature research.By searching the domestic and foreign databases of the literature related to the old people health management system,extract the content,and provide a theoretical basis for the formulation of in-depth interviews outline.2.Personal in-depth interviews.Ten old people living at home,nine caregivers and six health managers were interviewed.The interview data were analyzed by inductive method,and the content requirements of the three groups of people were integrated.On this basis,the first and second level items of the content of the old people health management system in the settlers were preliminarily drawn up.3.Delphi method.Delphi method was used to conduct two rounds of expert inquiries,and the contents of the health management system for the old people at home were determined according to the inclusion criteria of items(average score of importance assignment > 4.0,item selection rate > 80% and coefficient of variation < 0.2)and expert opinions.Results:1.The results of literature research show that the main contents of the geriatric health management system are health records,health assessment,health management reports,interactive communication platform,health information analysis and health knowledge.2.The main contents of the health management system for the old people at home,the caregivers and the health managers who participated in in-depth interviews are as follows: The health records include the basic information of the old people,collect the past history and record the physical examination data.In the part of health assessment,we should assess the diseases,common symptoms,self-care ability,psychological status,social support,home environment,health literacy and health risk of the old people.Health management report should include summary of main problems,disease risk prediction results,analysis results of adverse lifestyle,abnormal summary of physical examination items,and daily health advice.Interactive communication platform should include advance booking,messages in the system,community lecture notice,reminder of medical examination time and system customer service.Health information analysis should include personal health status change analysis and group health status change analysis;health knowledge should include the introduction of common geriatric diseases,common geriatric medication knowledge,diet-related knowledge of the old people,geriatric sports knowledge,geriatric mental health knowledge.Finally,interviewees believe that the effect evaluation should be supplemented in the system,including the evaluation of health management effect,health management intervention implementation and system satisfaction.3.A total of 27 experts were consulted from three fields: health management,geriatric nursing and geriatrics.The positive coefficients of experts in both rounds were 100%,and the authoritative coefficients of experts were 0.82 and 0.83,respectively.The Kendall’s W coefficients of the first and second entries in the first round were 0.239 and 0.267,respectively.And the Kendall’s W coefficients of the first and second entries in the second round were 0.301 and 0.353,respectively.After two rounds of expert inquiries,the health management system for the old people at home consists of seven first-level items and 29 second-level items.Conclusion:This study constructed a health management system for the old people at home,including 7 first-level items and 29 second-level items,such as health records,comprehensive assessment,health management reports,health management records,health analysis and health guidelines and effectiveness evaluation.The results of this study are scientific and reliable,can provide better content support for the development of the system.
