| Objective:To study the application of the standardized diagnosis and treatment in routine clinical practice of Non-small cell lung cancer and analyze if the standardized diagnosis and treatment was benefit for survival.Investigate the influences of the application of the standardized diagnosis and treatment.Methods: Complete clinical data of 369 cases of new diagnosed Non-small cell lung cancer patients were collected from The First Affiliated Hospital of Hainan Medical College between January 2012 and October 2016.Clinical staging was performed through AJCC 7th Edition of lung cancer staging.The guideline of lung cancer of CSCO(2016 Edition)was used to assess if the diagnosis and treatment was standardized.Results: According to the data collected,standardized diagnosis was detected in 249(67.5%)patients,while non-standardized diagnosis was observed in 120(32.5%)patients.Nonstandard handwriting 99(82.5%)by the physicians and wrong staging21(17.5%)are the main factors for the non-standardized diagnosis.Standardized treatment was detected in 152(41.9%)patients,while non-standardized diagnosis was observed in 217(58.1%)patients.Our statistical analyses showed that whether the patients were standardized treated was correlated with Standardized diagnosis,clinical staging and medical departments(p <0.001),While standardized treatment between the age,gender,pathogenic type,smoking history had no statistical significance differences.We found that standardized treatment were benefit for the survival of the patients with non-small cell lung cancer by Multivariate Cox Regression analysis(P < 0.001).And standardized treatment can significantly prolong life survival of stage I-IIIA and stage IV NSCLC patients by stratified analysis(P<0.001),while whether standardized treated in the stage IIIB NSCLC patients had no statistical significance differences.Conclusion: Non-standardized treatment had be common in the patients with NSCLC in our clinical practice.Standardized diagnosis,clinical staging and medical departments are correlate with the standardized treatment.Standardized treatment can significantly prolong life survival,especially the stage I-IIIA and stage IV NSCLC patients. |
