ObjectiveExplore the value of heart murmurs,abnormal transcutaneous oxygen saturation(SpO2),dyspnea,and extracardiac malformation these four screening indicators for the screening of congenital heart disease.MethodsA total of 12003 live births born in the obstetrics department of the Affiliated Hospital of Jiangsu University from June 2013 to June 2018 were involved in this study.Pediatricians applied heart murmurs,abnormal SpO2,dyspnea,and extracardiac malformation these four screening indicators for all newborns to screen congenital heart disease and made assessment of their effectiveness.The findings of color echocardiography which was performed by ultrasound cardiologists on all new borns involved in the study were used for the diagnosis of congenital heart disease,and identifying the specific type.Results(1)In this study,a total of 12003 newborns were involved,and 116 children were dected with congenital heart disease.The top three types of congenital heart disease were ventricular septal defect(57 cases),atrial septal defect(28 cases),and patent ductus arteriosus(16 cases).(2)Among all the children,there were 176 neonates with heart murmurs,80 of whom suffered from congenital heart disease;73 neonates with abnormal SpO2decline,30 of whom suffered from congenital heart disease;224 neonates with dyspnea,31of whom suffered from congenital heart disease;62 neonates with extracardiac malformation 8 of whom suffered from congenital heart disease.In this study,there were 229 neonates with heart murmurs or/and abnormal SpO2,94 of whom had congenital heart disease,when heart murmurs or/and abnormal SpO2 or/and dyspnea these three indicators were combined,397 neonates were screened positive,100 of whom had congenital heart disease,when heart murmurs or/and abnormal SpO2 or/and increased breathing or/and extracardiac malformation these four indicators were combined,448 neonates were screened positive,103 of whom had congenital heart disease.When combined indicators were used for screening,heart murmurs or/and abnormal SpO2,heart murmurs or/and abnormal SpO2 or/and dyspnea,three indicators combined,heart murmur or/and abnormal SpO2 and/or dyspnea or/and extracardiac malformation,four indicators,the sensitivity was 81.03%,86.21%,and 88.79%respectively.ConclusionCompared with heart murmurs or/and abnormal SpO2,heart murmurs or/and decreased percutaneous oxygen saturation or/and dyspnea or/and extracardiac malformation,these four indicators combined for the screening of congenital heart disease,the sensitivity of screening is increased,and the rate of missed diagnosis is decreased.The method is simple and easy to operate,non-invasive and economical,and has a promotional value.
