Retrospective Analysis Of Application Of Submental Island Flap For Oral And Maxillofacial Malignant Tumors

Objective: The submental island flap is located in the head and neck and maxillofacial lymph node drainage area I,so there is controversy about whether the oral and maxillofacial malignant tumor can be used to repair the defect.Using the patient data in the department to summarize and analyze the application conditions of the submental island flap for repairing maxillofacial defects in patients with malignant tumor.Methods: A total of 40 patients with defects after a resection of malignant tumors repaired by submental island flaps from 2008 to 2018 were enrolled.The patient data were collected and get the patient’s operation date,number of follow-ups,live or not,with or without recurrence and TNM staging and other information,statistical summary,Statistical analysis was performed using SPSS 18.0 software,which was considered statistically significant at p < 0.05.Results: Of the 40 patients,24 survived,16 died,2 patients had recurrence,and approximately 54.2% of the lesions in the survival group were located in the tongue.There was a statistically significant difference between patients with lymph node metastasis(+)and lymph node metastasis(-)(p<0.05).And there was a statistically significant difference between patients with stage IV recurrence and early-stage patients(p<0.05).But there was no significant correlation between the location of the lesion and the maximum diameter of the tumor and the recurrence of the tumor(p>0.05).Conclusion: main limitation of the submental island flap in the treatment of good cervical lymph node dissection and accurate flap preparation is lymph node metastasis(+)and clinical stage IV.Under the condition of negative lymph node metastasis,the infraorbital artery island flap can be used as a good and reliable choice for repairing oral and maxillofacial defects.Under the condition of positive lymph node metastasis,the application of the infraorbital artery island flap should be analyzed in combination with specific clinical conditions.In patients with stage IV clinical stage,the flap was used to repair the defect on the premise of relieving the symptoms of the patient and restoring the function of the patient.
