OBJECTIVE: To investigate how the plasma copeptin level joint point-of-care test cTnI level values in the early diagnosis of AMI and its relationship with the severity of coronary artery disease.METHODS: A total of 82 patients with acute chest pain within 12 hours who were admitted to the ER room of our hospital from December 2017 to September 2019 were enrolled.The clinical data were divided into acute myocardial infarction(AMI)group,unstable angina(UA)group and healthy group.The clinical data of the 30 healthy people were collected in the physical examination center of our hospital as the control group.The general data and laboratory test results among the three groups were compared to analyze the diagnostic value of copeptin level for AMI.The coronary angiography were performed to the patients diagnosed with AMI after admission and were divided into severe coronary atherosclerosis group(GS≥50,n=20)and mild coronary atherosclerosis group(GS<50,n=20)according to Gensini score.The Grace score within 24 hours of admission was calculated to further assess the relationship between copeptin level along with other laboratory parameters and the severity of AMI.RESULTS: White blood cell count,neutrophil count,platelet count,cTnI level,and copeptin level were significantly different among the AMI group,the UA group,and the control group.The copeptin level of the AMI group was significantly higher than that of the UA group and the control group(P < 0.05).The ROC curve showed that the early diagnosis value of copeptin for AMI was higher than that of cTnI,and the diagnostic value of copeptin joint cTnI was higher than that of single test.Further analysis of ROC curve showed that the sensitivity of copeptin is 90.48% and the negative predictive value is 92.73% at the optimal cutoff value of 13.980 pmol/L.Copeptin is an independent risk factor for predicting severe coronary atherosclerosis and is positively correlated with the severity of coronary artery disease(r=0.470,P=0.002).Conclusion: Copeptin level joint point-of-care test cTnI level is of high value in the early diagnosis of acute myocardial infarction.Copeptin level is an independent risk factor for the assessment of the severity of coronary atherosclerosis.The level of Copeptin is positively correlated with the severity of coronary artery disease. |
