Objective: To investigate the status of maternal breastfeeding and the self-efficacy of breastfeeding in the third trimester of pregnancy,analyze the influencing factors of breastfeeding self-efficacy,and explore the guidance of breastfeeding guidance for cesarean section,breastfeeding self-efficacy,first breastfeeding success rate and The effect of exclusive breastfeeding rate during hospitalization provides a reference for the use of narrative care to improve breastfeeding levels,extend breastfeeding time,and develop nursing measures related to maternal and child health.Methods: This study was implemented in two parts.The first part: 231 pregnant women in the third trimester who were admitted to the hospital in Lanzhou City from June 2018 to December 2018 were selected as subjects.The pre-study research tools are general data questionnaire,breastfeeding self-efficacy scale(BSES),breastfeeding knowledge questionnaire,investigating breastfeeding knowledge and self-efficacy status of breastfeeding,and analyzing the influencing factors of breastfeeding self-efficacy.The study provides a baseline for comparing baseline data and developing interventions.The second part: 94 cases of cesarean section women who met the inclusion and exclusion criteria were selected from the previous survey subjects.The random number table method was divided into intervention group and control group,47 cases each.The control group was given routine breastfeeding guidance.The intervention group implemented the guidance of breastfeeding for narrative care,and implemented breastfeeding knowledge and skills-related education on the basis of narrative empathy to provide information and emotional support for pregnant women.The maternal breastfeeding status of the two groups was investigated after the first breastfeeding,and the success rate of breastfeeding was compared between the two groups.On the fourth day after surgery,the maternal self-efficacy scores of breastfeeding and the exclusive breastfeeding rate during hospitalization were compared.Statistical differences were used to assess the effectiveness of narrative care breastfeeding guidance.Results: 1.Cross-sectional study results: 222 valid questionnaires were included in the study analysis.The maternal breastfeeding knowledge scores in the third trimester were (9.49±1.76)and the breastfeeding self-efficacy scores were(91.00±10.78),both in the low-level,single factor.And related analysis results showed that breastfeeding selfefficacy and nipples have adverse conditions,expected delivery methods,planned breast milk duration,whether there is concern about postpartum milk deficiency,breastfeeding decision time and breastfeeding knowledge level,there is statistical difference(P<0.05)The results of multiple linear regression analysis showed that breastfeeding knowledge,nipples have adverse conditions,expected delivery methods,planned breast milk duration,whether there is concern about postpartum milk deficiency,and breastfeeding decision time are independent influencing factors of breastfeeding self-efficacy.2.Randomized controlled intervention study results: Except for sample shedding,46 patients in the intervention group and the control group were included in the study.There was no statistically significant baseline data,breastfeeding knowledge and breastfeeding self-efficacy scores of the two groups before the intervention(P >0.05).After intervention,the maternal breastfeeding knowledge,total breastfeeding selfefficacy score,first breastfeeding success rate and exclusive breastfeeding rate during hospitalization were statistically significant(P<0.05).Among them,the first breastfeeding success rate was breast milk.The feeding measurement tool score was used as a reference.The difference between the sucking condition of the baby in the breastfeeding measurement tool and the posture dimension of the baby with the nipple was statistically significant(P<0.05),the timing of feeding and the baby’s exposure to the breast.There were no significant differences in the performance dimensions between the two groups(P>0.05).There were significant differences in the skill dimension and the inner activity dimension between the two groups(P<0.05).Conclusion: Breastfeeding knowledge,whether nipples are in poor condition,expected mode of delivery,planned breastfeeding duration,concern about postpartum milk deficit and breastfeeding decision time are the main influencing factors of maternal breastfeeding self-efficacy in the third trimester;applying narrative care to cesarean section Postoperative breastfeeding guidance can improve maternal breastfeeding knowledge,breastfeeding self-efficacy,first breastfeeding success rate and exclusive breastfeeding rate during hospitalization. |
