| Objective To analyze the relationship between the urinary iodine concentration and thyroid stimulating hormone(TSH),thyroid autoantibodies and thyroid nodules in adults in parts of Gansu.Method A total of 2081 people who participated in the National Survey of Thyroid Diseases and Iodine Nutrition-2014(Tide)were selected according to the research conditions.The subjects were divided into four groups: iodine deficiency group:UIC<100μg/L;iodine sufficient group: UIC 100-199μg/L;iodine super sufficient group: UIC 200-299μg/L;Iodine excess group: UIC≧300μg/L.The puepose of this research was to analyze the relationship between different urinary iodine levels and serum TSH levels,thyroid peroxidase antibody(TPOAb)levels,thyroglobulin antibody(TGAb)levels and their positive rates,and thyroid nodules prevalence.Results(1)The levels of TSH in females UIC100-199μg/L group,UIC200-299μg/L group and UIC≧300μg/L group were significantly higher than those in male group(P<0.001).There was no difference in TSH among men and women with different urinary iodine levels(P>0.05).(2)The prevalence of TSH≧4.2mIU/L and TSH≦0.27mIU/L in female UIC100-199 μg/L group and UIC200-299 μg/L group was significantly higher than that in males(P<0.05).(3)Compared with UIC100-199μg/L group,the levels of TPOAb and TGAb in UIC<100μg/L group were significantly lower(P<0.05);the levels of TPOAb and TGAb in UIC≧300μg/L were significantly higher(P<0.05).In the same iodine group,the levels of TPOAb and TGAb in women were significantly higher than those inmen(P<0.001).(4)The positive rate of females’ TPOAb in UIC<100μg/L group,UIC100-199μg/L group and UIC200-299μg/L group was significantly higher than that of males’(P<0.05);The positive rate of females’ TGAb in UIC100-199μg/L,UIC200-299 μg/L group and UIC≧300μg/L group was significantly higher than that in males’ groups(P<0.05).(5)The prevalence of thyroid nodules and the thyroid nodule diameter >1 cm in the UIC<100μg/L group were significantly higher than those in the UIC100-199μg/L group(P<0.05);The prevalence of thyroid single nodules and multiple nodules in the UIC<100μg/L group and UIC≧300μg/L group was significantly higher than that in the UIC100-199μg/L group(P<0.05).In the same iodine group,the incidence of multiple nodules and nodules’ diameter>1cm in women is significantly higher than that in men(P <0.05).(6)Women are independent risk factors for thyroid nodules(OR:1.433,P=0.001);age,TSH and thyroid nodules are positively correlated(OR:1.062,1.062;P:0.000,0.002).And urinary iodine is negatively correlated with thyroid nodules within a certain range.Conclusion(1)When the UIC<100μg/L or UIC ≧ 300μg/L,serum TSH levels can be increased,and the prevalence in females is higher than in males.(2)UIC ≧ 300μg/L can lead to increased levels of thyroid autoantibodies and positive rates,and the levels and positive rates in females are higher than in males.(3)The condition of UIC<100μg/L or UIC ≧ 300μg/L can increase the prevalence of single nodules and multiple nodules in the thyroid gland,and the prevalence of women is higher than that of men. |
