Influence Of Infrapatellar Fat Pad On Joint Function After Primary Total Knee Arthroplasty

Objective: The paper is to study the influence of reservation and total resection of Infrapatellar fat pad(IPFP)on joint function when operating Total knee arthroplasty(TKA).Methods:Data of 64 patients conducted TKA in Affiliated Hospital of Qinghai University from December,2016 to December,2017 were selected as study object,and being divided into Group A(IPFP reserved)and Group B(IPFP resected).Group A consists of patients(aged from 58 to 78,66 on average)both 11 males and 21 females,among which 17 left knees and 15 right knees were operated.While Group B covers patients(aged from 55 to 77,65 average)both 13 males and 19 females,among which 19 left knees and 13 right knees were operated.With KTA performed by the same surgeon,patients were treated and guided by the same rehabilitation exercise program later,the data before and after KTA operation including activity of knee,HSS function score,Install-Salvati(IS)ratio were collected,other corresponding data including duration of operation,amount of blood loss during operation,amount of post-operation drainage and hospitalization duration were documented,to analyze the postoperative influence of reservation and total resection of IPFP on joint function,through conducting statistical process on data.Results:In the aspect of activity of knee and HSS function score,the difference within each group has a statistical significance(P<0.05),while the statistical difference between two groups is not obvious(P>0.05).Besides,in Group B(IPFP resected),Length of patella tendon(LPT)decreased from 55.54±1.62 mm before operation to 48.60±2.10 mm after operation,Insall-Salvati(IS)ratio decreased from 0.99±0.02 to 0.89±0.04.In comparison,the difference of post-operation drainage between two groups(Group A 152.81±115.08 ml,Group B 180.78±13.08ml)has a statistical significance(P<0.05).Conclusions: In TKA operation,satisfactory curative effect in activity of knee and HSS function score can be achieved through both reservation and total resection of IPFP.But the Group B(IPFP resected)gets a lower Patellar height,a shorter Length of patella tendon(LPT),and increased post-operation drainage,compared with Group A(IPFP reserved).
