Measurement Of Common Bile Duct In Healthy Children With Ultrasonography And Its Application In Diagnosis Of TypeⅠcongenital Biliary Dilatation

Objective 1.To determine the 95% reference value range inner diameter of common bile duct in healthy children with ultrasonography measurement and analysis correlation between age,BMI,waistline and inner diameter of common bile duct;2.To discuss the value that measurement of common bile duct for diagnosis typeⅠcongenital biliary dilatation.Methods 1.Measurement of common bile duct in aged 0~12 years healthy children with ultrasonography,and recorded measured data and general information,include age,gender,height,weight,waistline.2.Then we collected and recorded clinical data of typeⅠCBD children aged 0-12 years,include age,gender,clinical manifestations and imaging examination.All cases were confirmed by surgical pathology.The result and conclusion were obtained through statistical analysis.Results 1.Different age 0~1year,1~3years,3~7years,7~10years,10~12years 95%reference value range of common bile duct inner diameter respectively were: portal hepatis segment1.6mm,1.9mm,2.7mm,3.3mm,3.6mm;mid-segment 1.9mm,2.5mm,3.0mm,3.9mm,4.2mm;pancreas segment 1.3mm,1.4mm,1.9mm,2.2mm,2.4mm.We detect there was a positive correlation between age and portal hepatis segment,mid-segment,pancreas segment common bile duct inner diameter(r=0.790,r=0.806,r=0.639;P<0.05);there is a weak correlation between BMI and portal hepatis segment,mid-segment,pancreas segment common bile duct diameter(r=0.255,r=0.263,r=0.207;P<0.05),there is a positive correlation between Waistline and portal hepatis segment,mid-segment,pancreas segment common bileduct diameter(r=0.542,r=0.565,r=0.296;P<0.05);there was no statistically difference in the common bile duct between the two genders.2.Most of the CBD children patients in this group were female(76%),and it mostly concentrated at 0-3 years old(64%).The right upper abdominal mass and jaundice were the main causes for the admission of CBD patients aged0-3years.CBD patients aged 4-8years were mainly treated with abdominal pain,fever and vomiting.We conclude the cutoff of the common bile duct was 4.95 mm,the sensitivity and specificity were 71.4%,83.6%.Conclusion 1.We determined common bile duct inner diameter reference value in children aged 0-12 years,if exceed the upper limit should alert common bile duct dilatation.There are positive correction between age,BMI,waistline and inner diameter of common bile duct.there was no statistically difference in the common bile duct between the two genders.2.There is high diagnosis value for CBD to measurement the inner diameter of common bile duct with ultrasonography.
