| Objective Based on researching into the osteoporotic fracture patients(2013-2017 Ningxia Medical university general hospital clinic)follow-up survey,fracture patients awareness of Osteoporosis(Osteoporosis,OP)definition,need treatment(with the exception of foundation treatment)awareness,drug therapy and non-drug therapy awareness,etc.,as well as the awareness of follow-up,in patients with Osteoporosis special cognitive situation,such as checking method of the results with previous research results,early in 2008-2012years)to the research result is compared,in order to understand the improvement of patients’ knowledge of osteoporosis.Through the questionnaire survey of doctors in different departments of the general hospital of Ningxia Medical university,the medical staff’s understanding of osteoporosis fracture and the improvement of prevention and management were obtained.The average hospitalization cost,re-fracture status and re-fracture cost of the patients with osteoporosis fractures in recent 5 years were obtained.Methods The general hospital of Ningxia Medical university provided the list of cases and basic information of inpatient diagnosis and treatment according to the inclusion criteria.Telephone follow-up(home visit if necessary)was conducted to investigate the prognosis and treatment of patients after discharge(500 cases of hip fracture and 750 cases of spinal fracture admitted to general hospital of Ningxia Medical university,a total of 1,250 cases).A questionnaire survey was conducted among doctors in departments related to osteoporosis inhospital of Ningxia Medical medical university(a total of 750 cases of medical staff in different departments related to osteoporosis were investigated).Use a unified questionnaire and information sheet,I personally undertake the investigation.Results A total of 1250 teleph one questionnaires were consulted,including 38 deaths,121 lost visits,1091 effective ones and 87.28% effective recovery.A total of 750 questionnaires were issued to medical staff B in the departments related to osteoporosis.Results: 1)the awareness rate of osteoporosis is 51.4%,the awareness rate of risk is32.7%,the awareness rate of needing treatment is 12.0%,and the awareness rate of needing medical treatment is 7%.The awareness rate of follow-up was 5.7%.2)the awareness rate of biochemical indicators to be tested is 2.3%,the awareness rate of vitamin D to be tested is2.8%,and the awareness rate of bone conversion indicators to be tested is 1%.3)the awareness rate of the detection indicators requiring follow-up for patients with osteoporosis after treatment was 1.9%,the awareness rate of the follow-up time for patients with osteoporosis after treatment was 0.5%,the awareness rate of the movement mode in the treatment for patients with osteoporosis was 1.6%,and the awareness rate of the dietary treatment for patients with osteoporosis was 2.9%.4)awareness of osteoporosis patients need calcium is 99.7%,the awareness of osteoporosis patients need vitamin D treatment was 29.1%,patients with osteoporosis bone pain when using calcitonin witting rate was 1.2%,osteoporosis patients need bisphosphonate therapy witting rate was 1.6%,and osteoporosis patients for the adverse effects of calcitonin treatment witting rate was 1.5%,and for oral bisphosphonate therapy in patients with osteoporosis witting rate was 0.9%,the side effects of osteoporosis patients for intravenous use bisphosphonate treatment side effects of the awareness of 13.6.All of the above data calculated by SPSS17.0 chi-square,and areas of Ningxia Medical hospital five years before,comparing the results according to the results of osteoporosis patients with osteoporosis concept,risk,need medication(especially calcium and vitamin D treatment),to detect biochemical indicators need to be detected,vitamin D,diet therapy,ways of bisphosphonate therapy and follow-up awareness,awareness from the previous have improved significantly(p < 0.05,with statistical significance).However,there was no significant change in the patients’ recognition of osteoporosis including bone conversion index,calcitonin,calcitonin in the case of bone pain,follow-up time,and osteoporosis exercise pattern(p>0.05,no statistical significance).Among them,281 OP patients were re-fracture after osteoporosis fracture,and the re-fracture rate reached 25.8%.The average hospitalization cost of the 1,091 osteoporosis patients surveyed was about 28,500,and the cost of re-fracture was more than double that of primary fracture.Ningxia Medical university general hospital medical staff survey results: 1,the understanding of the general cognition and diagnosis of OP,OP concept witting rate was98.6%,the diagnostic method witting rate was 69.3%,the susceptible crowd witting rate was45.4%,the types of medication witting rate was 31.0%,follow-up way witting rate was 7.6%;2.Cognition of special OP test methods: awareness rate of biochemical indexes needed to be tested was 14.4%,awareness rate of vitamin D needed to be tested was 72.1%,and awareness rate of bone conversion indexes needed to be tested was 6.8%;3.Cognition of medical staff on OP follow-up and non-drug treatment: awareness rate of follow-up test index was 3.5%,awareness rate of follow-up time was 3.9%,awareness rate of sports treatment prescription was 1.7%,awareness rate of diet treatment prescription was 2.5%;4.Cognition of OP medication: the awareness rate of calcium supplementation was 95.2%,that of vitamin D supplementation was 58.1%,that of calcitonin for bone pain was 4.4%,and that ofbisphosphonate therapy was 14.7%.5,the OP cognitive cases of adverse drug reactions: the adverse effects of calcitonin treatment witting rate was 3.8%,and the side effects of oral bisphosphonate witting rate was 3.4%,and venous side effects of using two phosphate treatment witting rate was 5.2%,all of the above data by chi-square test of SPSS to calculate,and the xxx region for five years before the hospital medical staff,comparing the results according to the results of osteoporosis related department staff approach to the diagnosis of osteoporosis,susceptible population and kinds of drug treatment of cognitive is improved from the previous,the definition of osteoporosis,with no obvious improvement.The awareness that OP needs to be tested for biochemical indexes and OP needs to be tested for vitamin D has been improved,while the cognition of photon bone density measurement,symptoms and signs and OP needs to be tested for bone conversion indexes has not been improved.The cognition of OP follow-up,dietary prescription of anti-op drugs and non-drug treatment,vitamin D and bisphosphonate was improved,while the cognition of follow-up test index,follow-up time,exercise therapy prescription,calcium supplement and calcitonin for ostealgia was not improved.Conclusion Ningxia Medical university general hospital clinic patients with osteoporotic fractures related to osteoporosis awareness and understanding,after the January2008 to December 31,2012,after the research about the society,media intervention and hospital organization knowledge of osteoporosis related to preach became popular,people of osteoporosis in patients with significant correlation recognition improved,but there are still insufficient,of osteoporosis in patients with certain special inspection method and non-drug,still need to further strengthen consciousness of drug therapies.The understanding of osteoporosis and the management knowledge of the patients after fracture were better than before.Therefore,as a medical staff,in the future clinical road,weshould strongly advocate large knowledge dissemination,community or treatment guidance during hospitalization,so that patients can improve the concept of medical treatment,early diagnosis,early treatment,early recovery,improve the quality of life of patients,reduce the family and social and economic burden.At the same time,medical workers are encouraged to continuously improve their professional knowledge level in the future work,which can remove the disease for patients to the greatest extent. |
