At present,China's medical and health undertakings are facing many challenges.The phenomenon and contradiction of population aging,urbanization and high incidence of chronic diseases are introduced.The existing medical and health service model can not meet the needs of community residents.The hospital and disease-centered service model can not effectively solve the problem.Primary medical and health service system plays an important role in the medical and health services,and has attracted widespread attention from all walks of life.In developed countries,family doctors are one of the dreams of community residents,an important part of the primary health care system,to have their own family doctors.As an important attempt of health care reform,community health management has become a very important basic link.The promotion of family doctors is an important move.Based on the field investigation and questionnaire survey of community health service centers in Wenjiang District of Chengdu,this paper analyzes the problems and causes of family doctor service in community health service centers,and finds the following problems in the promotion of family doctor contract service in Wenjiang District: 1.Family doctors lack of awareness,publicity and details.Only 6.4% of the residents knew the contents of the family doctor,26.1% knew the contents of the family doctor.2.Medical staff did not know enough about the contracted services of the family doctor.There are significant differences between general practitioners and other medical staff.3.The supporting policies are not perfect and the effective service model is not formed.Residents' signing rate is not high and their performance is not strong;4.The lack of incentives is the main influencing factor;5.The implementation of work objectives is more difficult,less interaction with the contracted residents;6.The lack of effective supply,demand side demand positioning is not clear,service demand is not released 7.The effective supply of talent is insufficient,it is difficult to satisfy.Sufficient actual demand;8.,informatization construction is backward,affects work efficiency,increases work content.In view of the problems existing in the implementation of contract service for family doctors in Wenjiang community of Chengdu,this study proposes the following countermeasures: 1.Perfect the incentive mechanism for hospitals,family doctors and residents at all levels,construct the four-level performance appraisal system of city-district-institution-family doctor team,and draw lessons from the practice of "disease diagnosis and treatment classification standard" in the United States.2.Establish a training base for family doctors,build a joint publicity network in the streets,and adopt a "snowball" type of rotation training by means of teachers and apprentices.3.Establish the contract service fee system,through public health funds,adopt the mode of paying contract service fee according to the number of contract signers,explore the establishment of a head-to-head medical payment system,pay attention to the establishment of a family doctor team assessment mechanism;4.Improve the medical staff salary and welfare system,strengthen the construction of general practitioners,establish and improve the system.The training system will promote general medical education in medical colleges and universities.Increase source supply,maintain the professional quality of the team 5.make full use of "Internet +" to promote the informatization construction of family doctor contract service,and ensure the continuity of residents' health records through the construction of information technology,liberate human resources and improve work efficiency.Finally,policy safeguard measures are put forward.
