| Objective This study aimed to understand the relationship of mild cognitive impairment and associated factors through investigating and analyzing the general data of the elderly in Shanghai community.At the same time,comparing the performances of individuals on the Neuropsychological Test Battery to explore the valuable cognitive indicator of early effective identification of MCI.Methods This study adopted the case-control study to select 267 MCI patients as cases group(MCI group)and 710 normal elder as normal control group(NC group).Face-to-face interview was conducted to collect relevant information with prepared questionnaires by specially trained investigators and assessed by the Neuropsychological Test Battery.The NTB comprises the following cognitive tests of various cognitive domains:the Digit Span(forward and backward span length),Auditory Verbal Learning Test,Associative Learning Test,Visual Recognition Function Test,Verbal Fluency Test,Wechsler’s Mapping,and Wechsler’s Block Diagram.χ~2analysis,t test,binary forward Logistic regression analysis and ROC curve analysis were performed to evaluate associated factors with cognitive impairment and the application of AVLT diagnosing MCI.Results There were significant differences between age(t=-11.3,P=0.000),gender(χ2=26.0,P=0.000),education(t=11.2,P=0.000),occupation(χ2=45.0,P=0.000)、smoking(χ2=10.5,P=0.001),drinking tea(χ2=12.1,P=0.002),hobbies(χ2=22.7,P=0.000),including reading(χ2=24.0,P=0.000)and surfing the internet(χ2=10.6,P=0.000),eating fish(χ2=4.3,P=0.041),middle age the average night of sleep time(t=2.6,P=0.031),diastolic blood pressure(t=2.2,P=0.031),weight(t=4.6,P=0.000),a variety of physical complained(χ2=15.3,P=0.000)、history of disease(χ2=10.3,P=0.016)and a diagnosis of other diseases(χ2=4.9,P=0.028)between MCI group and normal group.The results of binary forward regression logistic analysis are that MCI was influenced by age,education,a variety of physical complained and a diagnosis of other diseases.The protective factor was high education(OR=0.798,CI=0.740~0.861)and the risk factors of MCI including age(OR=1.084,CI=1.038~1.132),a variety of physical complain(OR=3.428,CI=1.185~9.920)and a diagnosis of other diseases(OR=2.045,CI=1.088~3.845).The results of NTB tests demonstrated that there were significant differences between the two groups in seven of the subtests at 0.01 level besides AVLT-5(t=-0.071,P=0.946).The AUC(Arear under curve)of MoCA total score was 0.897(CI=0.873~0.920),the AUC of NTB total score was 0.866(CI=0.839~0.893),and the AUC of MMSE total score was 0.859(CI=0.829~0.899).And we got the results were that AVLT1 score,association learning test and verbal fluency test to discriminate MCI from NC were 0.81(CI=0.783~0.846),0.803(CI=0.767~0.840)and 0.793(CI=0.759~0.827)in the same way.Conclusions Our findings suggest that the protective factor is high education and the risk factors of MCI including age,a variety of physical complain and a diagnosis of other diseases.NTB total score and subtest score,especially the AVLT score,could discriminate MCI from normal controls and thus are sensitive tools in early detection of MCI. |
