Objective: The research collected data on age,history of hypertension and history of type 2diabetes from patients with cerebral small vessel disease(CSVD)and normal controls,and detected the level of the 25-hydroxyvitamin D(25-OH-VD),glucose(GLUI),urea(UREA).),creatinine(CR),uric acid(UA),triglyceride(TP),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C)to analyze whether there is statistical difference between these risk factors in the case group and the control group,to explore whether there is a correlation between 25-OH-VD level and cerebral small vessel disease,and then to explore whether the reduction of 25-OH-VD is the risk factors for leukoaraiosis and lacunar infarction and whether the level is related to the severity of imaging manifestation.The research provides a guide for the etiology study of cerebral small vessel disease,and provide a theoretical basis for clinical individualized treatment.Methods: A total of 204 patients with cerebral small vessel disease admitted to the Department of Neurology,Baotou Steel Hospital of Inner Mongolia were selected as the case group.A total of 198 non-CSVD patients confirmed by radiographic examination in the neurology department were selected as the control group.According to the results of magnetic resonance imaging(MRI),the CSCD group was graded according to the number of lacunar infarction(LI)lesions and leukoaraiosis(LA)fazekas method.The patients with LI and LA were divided into three groups: light,moderate and severe.All subjects were collected for fasting venous blood,and biochemical indicators such as 25-OH-VD,GLUI,UREA,CR,UA,TP,TC,LDL-C,HDL-C were detected,and general information and past medical history of all cases were recorded and analyzed,screening for suspected risk factors for cerebral small vessel disease.Then the researchers made univariate and multivariatelogistic regression analysis on suspected risk factors,and analyzed the correlation between25-OH-VD and other factors.The Spearman correlation coefficient was used for hypertension,diabetes and the severity of LI and LA.The Pearson correlation coefficient was used for measurement data such as age.Results: The research collected a total of 204 patients with CSBD,including 173 patients with LI,with an average age of 70.05±5.041 years;156 patients with LA,with an average age of 70.17±4.807 years;and 198 patients in control group with an average age of69.38±4.135 years.By comparing the control group and the cerebral small vessel disease group,it was found that the level of 25-OH-VD in the cerebral small vessel disease group(11.48±2.902 ng/ml)was significantly lower than that in the control group(15.59±3.436ng/ml),and the proportion of hypertension(63.2%),the proportion of diabetes(48.5%),total cholesterol(4.64±1.235 mmol/L),and low-density lipoprotein cholesterol(2.66±0.381mmol/L)were significantly higher than those in the control group(p <0.05).According to the results of single factor analysis,researchers took patients with hypertension,diabetes,total cholesterol,low-density lipoprotein cholesterol,25-hydroxyvitamin D as independent variables,whether to have cerebral small vessel disease as a dependent variable,and make multi-factor logistic regression analysis.The study found that hypertension and the reduction of 25-OH-VD are the main risk factors of CSCD.The probability of developing CSBD,LI,LA in hypertensive patients is 2.767 times,2.617 times,3.424 times of normal individuals.The Spearman correlation coefficient was used for 25-OH-VD,hypertension and diabetes,and the Pearson correlation coefficient was used for age and other measures.The correlation coefficient between 25-OH-VD and age was-0.169,and its level was negatively correlated with age;The correlation coefficient of the factors is not significant.Spearman correlation analysis was performed on the 25-OH-VD and severity of LI group and LA group.The correlation coefficient between 25-OH-VD and LA severity was significant,and the correlation coefficient was-0.167,but the correlation coefficient between 25-OH-VD and severity LI.is not significant,and there may be no correlation between 25-OH-VD and the severity of Lacunar infarction.Conclusions:1)Hypertension,diabetes,elevated total cholesterol,elevated LDL cholesterol,and decreased25-hydroxyvitamin D are risk factors for cerebral small vessel disease.2)Hypertension and reduction of 25-OH-VD may be independent risk factors for lacunar infarction and leukoaraiosis.3)25-OH-VD is negatively correlated with age.4)The reduction of 25-OH-VD may be negatively correlated with the severity of white matter hyperintensities,and may not be related to the severity of lacunar infarction.
