Objective1.To understand the status quo of postpartum depression in elderly parturients.2.To explore the influencing factors of postpartum depression in elderly parturients.3.To analyze the relationship between postpartum depression,hope level and family function in elderly parturients.4.Understanding the causes of postpartum depression in elderly parturients in combination with qualitative research to make up for the lack of quantitative research results.MethodsQuantitative and qualitative research methods were used to investigate the influencing factors of postpartum depression in elderly parturients and their correlation with hope level and family function.1.Quantitative study is a cross-sectional survey study.204 elderly parturients who delivered in files in the Affiliated Hospital of Inner Mongolia Medical University from March 2018 to December 2018 were selected by convenient sampling method as the research object.The general situation questionnaire,Edinburgh Postpartum Depression Scale,Herth Hope Scale and Family Care Index Questionnaire were filled out 2-7 days after delivery to investigate the elderly maternal.Data were processed by statistical software SPSS25.0,and the results were analyzed by descriptive statistics,X~2test,Ztest,multiple stepwise regression and partial correlation analysis.2.Qualitative research is phenomenological research method,using purposeful sampling method.Sixteen representative samples were selected from the elderly parturients who had been included in the study group for interviews,and semi-structured interviews were used for interviews.Colaizzi's seven-step phenomenological analysis was used to code the interview data,refine the theme,and further explain the results of quantitative research.Results1.The median of postpartum depression in 204 elderly parturients was 9.Among them,31 elderly parturients scored more than 13 points for postpartum depression.The median of postpartum depression group was 13,and the incidence of postpartum depression was 15.20%.173 parturients scored less than 13 points for postpartum depression.The median score of postpartum depression in non-depression group was 9,and the number of non-depression group accounted for 84.80%of the total number.2.The influence of education level,postpartum sleep,infant birth,husband-wife relationship,payment method of medical expenses,average monthly family income,occupation and living conditions of elderly parturients on postpartum depression was significantly different.(P<0.05).Pregnancy and childbirth history,mode of delivery and sex of infants had no significant effect on the occurrence of postpartum depression in elderly parturients.3.Postpartum depression score was significantly correlated with hope level score(P<0.05).4.Postpartum depression score was significantly correlated with family care index score(P<0.05).5.The main influencing factors of postpartum depression in elderly parturients were their education level,infant birth status and average monthly family income.6.Qualitative research results6.1 Theme One::Lack of Basic Needs6.2 Theme two:Handling Obstacles to Stress Sources6.3 Theme three:The lag of role transformation6.4 Theme four:Lack of Material Support Conclusion1.The factors influencing postpartum depression are the education level of the elderly parturients,postpartum sleep,birth status of infants,marital relationship,payment of medical expenses,per capita monthly income of families,level of hope and family function.Among them,maternal education level,infant birth status and family per capita monthly income are the main influencing factors.2.Hope level and family function were negatively correlated with the occurrence of postpartum depression.The higher the level of hope and family function,the lower the incidence of postpartum depression.Therefore,the evaluation of the hope level of elderly parturients can effectively predict postpartum depression.At the same time,giving full family care to elderly parturients can prevent the occurrence of postpartum depression.3.Quantitative research and qualitative research are combined to study problems from multiple perspectives and aspects.The results of the study complement each other,suggesting that medical staff should find out the high risk factors of postpartum depression in elderly parturients in time,communicate with their families and intervene as early as possible,so as to reduce the incidence of postpartum depression and ensure the mental health of elderly parturients.
