Objectives To analyze the expression of nm23(non-metastatie,Gene cloning of plant23)and cyclooxygenase 2(COX-2)in papillary thyroid carcinoma(PTC),nodular goiter and normal thyroid,and to explore the relationship between the two proteins and the clinical features of papillary thyroid carcinoma.Explore the function of the two proteins in the occurrence an development of papillary thyroid carcinoma,thy to find higher specificity and sensitivity biological indicators for the early diagnosis,treatment and prognosis of papillary thyroid carcinoma.Methods 1 This experiment collected 119 cases of thyroid disease tissue resected and paraffin-embedded by head and neck surgery from Tangshan Workers’ Hospital from June2015 to December 2017.Including 69 cases of papillary thyroid carcinoma,30 cases of nodular goiter,20 cases of normal thyroid tissue.All patients with thyroid disease were newly diagnosed before surgery,and had no adjuvant treatment such as radiotherapy or chemotherapy,and the medical records were complete.2 The positive expression of nm23 and COX-2 protein in papillary thyroid carcinoma,nodular goiter and normal thyroid tissue was detected by immunohistochemical SP method.To analyze the expression of two proteins in thyroid papillary carcinoma with different clinical characteristics.To investigate the role of nm23 and cox-2 in the occurrence and development of thyroid papillary carcinoma and the interaction between them.Results 1 The positive expression rate of nm23 in the papillary thyroid carcinoma group was 28.99%,which was significantly lower than that in the nodular goiter and normal thyroid group(76.67% and 85.00%),the difference was statistically significant(P<0.05).2The positive expression rate of COX-2 in the papillary thyroid carcinoma group was71.01%,which was significantly higher than that in the nodular goiter and normal thyroid group(20.00% and 20.00%).The difference was statistically significant(P< 0.05).3 In papillary thyroid carcinoma,the positive expression rate of nm23 in the <55 years old age group was 28.57%,and the positive expression rate in the age group ≥55 years old was30.77%,the difference was not statistically significant(P>0.05).The positive expression rate of nm23 in male group was 20.00%,and the positive expression rate in female group was 32.65%.The difference was not statistically significant(P>0.05).The positive rate of nm23 in the single-shot group was 30.77% and the number of lesions was more frequent.There was no significant difference in the positive rate of 23.53%(P>0.05).The positive expression rate of nm23 was 37.78% in the tumor diameter ≤1cm group and 12.50% in the tumor diameter>1cm group.The difference was statistically significant(P< 0.05).the positive expression rate of nm23 in tumor invading gland capsule group was 8.70%,and the positive expression rate in glandular capsule intact group was 39.13%,the difference was statistically significant(P<0.05).The positive expression rate of nm23 in the lymph node metastasis group was 11.43%,and the positive expression rate in the lymph node metastasis group was 47.06%,the difference was statistically significant(P<0.05).The positive expression rate of COX-2 in the <55 years old group was 69.64%;the positive expression rate in the ≥55 years old group was 76.92%,the difference was not statistically significant(P>0.05).The positive expression rate of COX-2 in male group was 70.00%,and the positive expression rate in female group was 71.43%.The difference was not statistically significant(P>0.05).COX-2 was in the positive rate of single-shot group was71.15%,the positive rate of lesions was 70.59%,the difference was not statistically significant(P>0.05).COX-2 was in the tumor diameter ≤1cm group.The positiveexpression rate was 62.22%,and the tumor diameter>1cm group was 87.50%.The difference was statistically significant(P<0.05).The positive expression rate of COX-2 in the tumor invading gland capsule group was 95.65%,and the positive expression rate in the glandular capsule intact group was 58.70%.The difference was statistically significant(P<0.05).The positive expression rate of COX-2 in the lymph node metastasis group was85.71%,and the positive expression rate in the lymph node metastasis group was 55.88%.The difference was statistically significant(P<0.05).The positive expression levels of nm23 and COX-2 were not related to the clinical characteristics of the patient’s gender,age,and number of lesions,but were related to the size of the tumor,whether the lesion invaded the capsule and whether the lymph nodes metastasized.4 After Spearman correlation analysis,nm23 and COX-2 expression were negatively correlated(r=-0.296,P<0.05).Conclusions 1 The positive expression of nm23 protein in thyroid papillary carcinoma was significantly lower than that in nodular goiter and normal thyroid tissue.Tumor suppressor gene nm23 may play an inhibitory role in the occurrence and development of thyroid papillary carcinoma.The positive expression of COX-2 protein in thyroid papillary carcinoma was significantly higher than that in nodular goiter and normal thyroid tissue.COX-2 may play a promoting role in the progression of thyroid papillary carcinoma.2 The positive expression of nm23 and cox-2 showed no significant difference in the gender,age and number of lesions of patients with thyroid papillary carcinoma,respectively,but showed significant difference in the invasion of glandular capsule,tumor diameter and lymph node metastasis.3 There was a negative correlation between the positive expression level of nm23 and cox-2 in thyroid papillary carcinoma.There may be mutual inhibition between the two.4 The combined detection of nm23 and cox-2 may play a certain role in differentiating the benign and malignant thyroid tumors and the metastasis of thyroid papillary
