Objective This research aims to estimate the direct,indirect and total economic burden incurred by malignant tumor in a certain area of southwest China.Based on the “System of Health Accounts 2011 Edition”,the dimension of health expenditure is divided into the core dimension and extended dimension according to the three aspects of medical services: service function,fund-raising program and providers,to analyze the components of the economic burden,provide suggestions for the health department and discuss the effective measures for reducing the economic burden of malignant tumors.Methods This research adopts the multistage stratified sampling method and chooses healthcare and medical institutions of different levels and types to sort out and analyze the medical records of patients with malignant tumors.The direct medical expenditure from the direct economic burden caused by malignant tumor is calculated based on the “System of Health Accounts 2011 Edition”;the direct non-medical expenditure is calculated based on the data of the fifth National Health Service Survey;and indirect economic burden is measured with the human capital method.In the end,the total economic burden caused by malignant tumor is obtained.Results In 2016,the total economic burden caused by malignant tumor in this area is 18.862 billion yuan.The direct economic burden is 6.865 billion yuan and indirect economic burden 11.997 billion yuan.Specifically,direct medical expense is 6.4 billion yuan,accounting for 93.23%.From the point of view of service distribution,hospitalization expense account for the most in direct medical expense caused by malignant tumor,that is,5.259 billion yuan,accounting for 82.18%;in terms of expense flowing,the direct medical expense mainly goes to hospitals,with 6.094 billion yuan,accounting for 95.22%;in terms of disease types,the direct medical expense is mainly incurred by trachea,bronchus cancer,lung cancer,liver cancer and colorectal cancer.Conclusion The economic burden of diseases caused by malignant tumors is relatively serious.Specially the economic burden caused by trachea,bronchus cancer,lung cancer,liver cancer,gastric cancer and colorectal cancer in this area is more serious and with age increasing,the situation becomes much worse.Measures should be taken to enhance the tertiary prevention of malignant tumor,optimize financing structure,improve the ability of professional institutions,enhance the accountability of government,establish a special fund for malignant tumor and accelerate the promotion of medical insurance for major diseases and improve efforts in critical illness insurance.Figure2;Table24;Reference 61.
