Objective: To understand the composition of liver disease during pregnancy and to analyze the maternal and infant outcomes of liver disease during pregnancy.Methods: A total of 26,397 pregnancies were admitted to our hospital for three years.A retrospective statistical analysis was performed on 900 patients with liver damage during pregnancy.The composition ratio and the adverse effect on pregnancy were analyzed.Results:Among the 26397 pregnant women,900 were pregnant with liver disease,and the prevalence of liver disease during pregnancy was3.4%.The highest incidence of this data was 520 cases of Intrahepatic cholestasis of pregnancy(ICP),accounting for 57.7%,followed by 187 cases of liver damage caused by pregnancy-induced hypertension,accounting for 20.8%,and pregnancy with viral hepatitis(VHP)accounted for 12.5 %,among which chronic hepatitis B(HBV)is the most common,with 100 cases accounting for 12.2%.Acute fatty liver(AFLP)was only 10 cases,accounting for 1.1%.There were 14 cases of Hyperemesis gravidarum,accounting for 1.6%.There were fewer cases of pregnancycomplicated with hepatitis A viral(HAV),hepatitis C virus(HCV),autoimmune hepatitis(AIH),and drug-induced liver damage(DILI),including 1 case,2 cases,and 4 cases,2 cases,no HELLP cases.The main adverse outcomes of maternal and fetal liver disease during pregnancy are cesarean section,fetal distress rate,and premature birth rate.The type of gestational period and the cesarean section rate of gestational uterine shown below that the proportion of AFLP is 100% and the proportion of Severe ICP is 100%.mild ICP is moderate 84%,The proportion of Severe VHP is 90%,mild VHP is 33.3%,and Acs is 20%.Fetal distress: in turn,the proportion of AFLP is 70%,Severe VHP is 55%,gravity ICP is32.9%,mild ICP is 3.2%.Premature labor :The proportion of Severe ICP is37.6%,AFLP is 20%,Severe VHP is 40%,mild to moderate VHP,mild to moderate ICP and AsC induced premature rate separately were 14.4%,14.7%,16%.The incidence of maternal deaths was 10% for AFLP and 5%for Severe VHP.The incidence of perinatal death: AFLP is 20%,Severe ICP is 4%,Severe VHP is 10%,and other liver disease types did not have maternal and child death.There was no significant abnormality in the incidence of maternal and infant adverse outcomes in different pregnancy BMI,but the incidence of fetal distress and fetal death varied with gestational age and maternal age.Conclusion: The liver disease in pregnancy in our hospital is the most ICP,mainly increasing the cesarean section rate.AFLP and VHP had thegreatest impact on maternal and infant outcomes.The cesarean section rate,fetal distress rate,and maternal and infant mortality rates increased.The Biochemical Indicators and symptom of AFLP quickly recovered after the end of pregnancy.There was no correlation between BMI during pregnancy and the occurrence of adverse liver disease and adverse outcomes.As the gestational age and age increased,the adverse outcomes of maternal and infant increased.
