Objective:For the surgical treatment of non-muscular invasive bladder cancer,most clinical urologists recommend transurethral bladder tumor resection with less trauma and faster recovery.However,due to the high recurrence and progression of bladder cancer,some patients need to undergo secondary surgery or even multiple surgeries,which brings great economic burden and pain to patients.Therefore,how to improve the effect of the first operation has guiding significance for the treatment and prognosis of patients.The purpose of this study was designed to retrospectively analyze the rationality and advantages of transurethral plasma needle electrode cystectomy of bladder tumor and transurethral plasmakinetic resection of bladder tumor(PKRBT),so as to provide a more effective surgical method for urological surgeons and improve the prognosis and quality of life of patients.Method:The clinical data of 90 patients with non-muscle invasive bladder cancer who were admitted to our hospital from March 2014 to March 2018 were retrospectively analyzed.According to different surgical methods,the patients were divided into two groups.The needle electrode group had 41 patients and included 31 males and 10 females.The age ranged from 34 to 83 years old,with an average age of 60.8 years.Tumor sites included 33 cases of lateral wall tumors and 8 cases of non-lateral wall tumors.TNM staging included 32 cases of Ta stage and 9 cases of T1 stage.The diameter of tumor ranged from 0.5 to 3.5 cm,with an average diameter of 1.6 cm.The PKRBT group had 49 patients and included 38 males and 11 females.The age ranged from 31 to 83 years old,with an average age of 59.3 years.Tumor sites included 39 cases of lateral wall tumors and 10 cases of non-lateral wall tumors.TNM staging included 38 cases of Ta stage and 11 cases of T1 stage.The diameter of tumor ranged from 0.5 to 3.5 cm,with an average diameter of 1.5 cm.All Preoperative patients had undergone strict physical examination and had not undergone bladder tumor-related surgery.The operative time,intraoperative blood loss,the incidence of obturator nerve reflex,the rate of bladder perforation and 1-year recurrence rate after surgery were recorded and statistically analyzed.Results:The average operation time of the needle electrode group and the PKRBT group were 17.4±6.1 minutes and 17.1±5.3 minutes.The rate of intraoperative bladder perforation was 9.8% and 6.1%.The difference was not statistically significant(P > 0.05).The average intraoperative blood loss was 9.6±4.2 ml and 14.9±4.8 ml.The incidence of obturator nerve reflex was 2.4% and 20.4%.The rate of recurrence 1 year after surgery was 2.4% and 18.4%.The difference was statistically significant(P < 0.05).Conclusion:Transurethral plasma needle electrode is safe and effective in the treatment of non-muscle invasive bladder cancer,with advantages of controllable obturator nerve reflex,less intraoperative blood loss,Less intraoperative and postoperative complications and low postoperative recurrence rate.
