Syphilis Epidemiological Report Analysis And Clinical Prevention Strategy In Nanjing General Hospital

[Objective]This study is aimed at providing preventive strategies and safe surroundings for syphilis patients and patients with other diseases through the analysis of the epidemiological characteristics of syphilis in city Nanjing,as well as the investigation of awareness rate about syphilis-related knowledge among different groups in tertiary comprehensive medical institutions.[Method]Data Collection The epidemic data from 2015 to 2017 regarding city Nanjing was derived from the reporting information management system about infectious disease,under the Information System of Chinese Disease Prevention and Control,where epidemic data with regard to syphilis was reported by syphilitic disease report card offered by various medical institutions in Nanjing.According to the epidemic data,the time distribution,population distribution and spatial distribution characteristics of syphilis were analyzed.Questionnaire Design Appropriate questionnaires about the syphilis knowledge were designed for investigating the awareness rate of syphilis among clinicians.In terms of different subjects,the questionnaires were divided into two groups.The first group is the professional knowledge linked to syphilis,and the subjects are the medical staff.The second is the syphilis-related infection knowledge in daily life,and the survey targets are non-medical staff.A total of 120 questionnaires were distributed to different groups in three tertiary hospitals,including 32 doctors,28 nurses and 60 outpatients.Statistical analysis The syphilis epidemic database was established by SPSS 20.0 software based on the collected data.Then,statistical analysis was performed.The significance level of all statistical tests was defined as P<0.05.Statistical analysis was carried out on the status of the epidemic situation in Nanjing,and the understanding rate of syphilis knowledge by different groups respectively.[Result](1)Results of syphilis epidemic dataNanjing General Hospital has reported 7299 cases of syphilis from 2015 to 2017,accounting for 10.10%of the total number of reported syphilis cases in Jiangsu Province(72,301 cases).1667 cases are from Jiangning District with the proportion of 22.84%of the total in Nanjing.From the result of time distribution,it has obvious seasonal characteristics,and the incidence is lowest in February,just constituting 6.04%of the whole year.The main age of onset is 20-64 years old.Additionally,the incidence of women aged 15-44 is more than that of men.Reversely,the incidence of males in other age groups is more than that of females.Withal,the main group with high incidence is people working on housework or people unemployed,which takes up 36.68%of the total reported cases.52.30%of reporting units is from tertiary general hospitals,which is the highest.The main report type is recessive syphilis with the percentage of 46.91%.(2)Results of questionnaireThe overall awareness rates of the syphilis knowledge are not high among medical staff and non-medical staff,which are 68.47%and 54.09%respectively.The correct rate is irrelevant to occupation,training history,training level and frequency of medical staff.And the results are not statistically significant(P>0.05).However,the training method has an impact on the correct answer rate of the questionnaire,with a statistical significance(P<0.001).People who attended the lecture training have a higher correct rate than those who did not(71.61%and 45.83%respectively).The correct rates of three questions,"Can people suffer from syphilis after daily conversations with syphilis patients?Such as eating and shaking hands","Is a person who seems healthy supposed to be a syphilis patient?",and "Can syphilis recur after treatment?"are extremely low,only 3.33%,5.00%and 13.33%respectively among non-clinical persons.There are two questions with high correct rates,"Do yoincrease AIDS transmission possibility?"(95.00%and 91.67%).With regard to the knowledge of syphilis u know the transmission routes of syphilis?" and“Does syphilis among non-medical personnel,there is no statistically significant differences between gender,marital status,education level and occupation(P>0.05).[Conclusion]The situation of syphilis epidemic in Nanjing is generally stable.The high-incidence areas are mainly concentrated in suburbs,and the most type is recessive syphilis.The high-risk population is middle-aged women,and the incidence is lowest in February.Both the awareness rates of syphilis knowledge among medical staff and non-medical staff are not high.These epidemiological distribution characteristics and the awareness rate of syphilis knowledge suggest that the focus of syphilis prevention and treatment in Nanjing are the suggestions of clinical syphilis prevention strategies,the supervision of medical personnel training,and the education of the public.At the meantime,the improvement of the accuracy of syphilis diagnosis,and the establishment of a sound reporting system are of great importance.Strengthening the screening and treatment of special maternal groups and preventing occupational exposure of medical personnel are as well crucial.To sum up,establishing a joint management system for infectious diseases is conducive to the safe surroundings.
