| Objective:To observe the clinical effect of longdan xiegan decoction and wuwei disinfecting decoction in the treatment of acute pyelonephritis on shortening the course of treatment and reducing the time of antibiotic use,and to provide an effective method for the clinical diagnosis and treatment of acute pyelonephritis.Methods:A total of 86 hospitalized patients with standard acute pyelonephritis were included,and the patients were divided into treatment group and control group by the randomized controlled study method,with 43 patients in each group.The control group received conventional western medicine treatment,while the treatment group received traditional Chinese medicine longdan xiegan decoction combined with wuwei disinfection drink on the basis of western medicine treatment.A course of 10 days,the two groups respectively record the systemic effects of the two groups after treatment(chills,fever,body aches)and local area bladder irritation pain,waist pain,kidney taps pain symptoms disappear time,routine blood(white blood cells,neutrophils)and whole blood C-reactive protein(CRP)reduction,routine urine,the urine culture turn situation,evaluation of gentiana purging liver decoction and mixed disinfection treatment of acute pyelonephritis,shorten period of treatment,the clinical curative effect of time,reducing antibiotic use and observation in the two groups of patients in the treatment of adverse reactions.SPSS20.0 statistical software was used to conduct data statistics and evaluate its clinical efficacy.Results:(1)Comparison of curative effect of TCM syndromes: The total effective rate of the treatment group and the control group was 97.62% and 92.86% respectively,indicating that the curative effect of TCM syndromes in the treatment group was better than that in the control group.(2)Comparison of curative effect between two groups of western medicine: Thetotal effective rate of the treatment group and the control group was 95.24% and88.10% respectively,indicating that the curative effect of western medicine in the treatment group was better than that in the control group.(3)The disappearance time of systemic toxemia(fever,chills,soreness)was compared between the two group: Treatment of the two groups after effects of fever,chills,sore and ache all over the body disappear time,P < 0.05,the difference is statistically significant,the treatment group effects of fever,chills,sore and ache all over the body disappear time shorter than the control group,the treatment group in the early days to improve effects of fever,chills,sore and ache all over the body on the curative effect is better than the control group.(4)The disappearance time of bladder irritation,low back pain and percussion pain in the kidney area was compared between the two groups: Treatment of the two groups after bladder irritation,waist pain,renal area taps pain disappear time comparison,P values < 0.05,the difference is statistically significant,the treatment group the bladder irritation,waist pain,renal area taps pain disappear time is shorter than the control group,the treatment group improved in early bladder irritation,waist pain,renal area taps pain on the curative effect is better than the control group.(5)Comparison between the two groups of urine culture treatment after 7 days:Chi-square test was used to compare the number of subjects in the group whose urine culture turned negative after 7 days of treatment.P value < 0.05 was statistically significant,indicating that the curative effect of the treatment group was better than that of the control group in terms of early urine culture turning negative..(6)Blood routine and c-reactive protein values of the two groups after 3 days of treatment:White blood cell count,neutrophil percentage and C-reactive protein of subjects in the treatment group and the control group were compared before treatment and 3 days after treatment,and the differences were statistically significant(P<0.05),indicating that both groups could reduce white blood cell count,neutrophil percentage and c-reactive protein value.After 3 days of treatment,WBC count,neutrophil percentage and c-reactive protein value were compared between the two groups,andthe differences were not statistically significant(P >0.05).The results indicated that the two groups had similar efficacy in reducing white blood cell count,neutrophil percentage and c-reactive protein value.(7)Comparison of white blood cells and bacteria in urine routine after 3 days of treatment between the two groups:The chi-square test was used to compare the number of subjects in the two groups whose white blood cell count and bacterial count turned negative in the urine routine 3 days after treatment.The P values were all< 0.05,which was statistically significant,indicating that the treatment group was superior to the control group in the efficacy of early urine routine negative conversion.Conclusion:To sum up,for the treatment of acute pyelonephritis,basic treatment of western medicine combined gentian purging liver soup and drink mixed disinfection treatment,can improve the patients’ body toxemia,earlier bladder irritation,waist pain,renal area taps pain symptoms,improve routine urine situation earlier,and can turn earlier urine culture,so as to shorten the treatment course of acute pyelonephritis,reduces the cast time of antibiotic drugs. |
