Objective: to investigate the treatment method and effect of CGF bone increment and simultaneous implantation of HBIC implant in patients with alveolar bone deficiency.Methods:54 patients with bone deficiency treated in the oral and maxillofacial surgery implant center of the HeBei Medical University stomatological hospital from July 2012 to July 2018 have been followed up.All patients were treated by CGF bone increment and simultaneous implantation of HBIC implant.A total of 127 HBIC dental implants were placed,43 cases among them are treated by + HBIC simultaneous implantation in the anterior region of alveolar bone(including the first premolar).Paries lateralis approach of maxillary sinus floor lifting by the CGF bone increment + HBIC simultaneous implantation was performed in 11 cases.Over 6 years,we observed the bone tissue around the implants as index to evalute the clinical effect.Results:1.All the implants were repaired.Except one patient who suffered from implant fracture and crown loss due to improper use,the other patients achieved good functions after denture repair.The success ratio of the dental implant is 99.2%.2.A total of 43 patients with 98 HBIC implants were treated by CGF bone increment + HBIC simultaneous implantation in the anterior region of alveolar bone(including the first premolar).21 cases included the cone beam computer tomography(CBCT)images acquired in three time points,such as immediately after surgery,before and after crown restoration.The immediate median postoperative buccal bone mass(MPBBM)of these patients was 3.10±0.44 mm.And the data reduced to 2.75±0.40 mm before crown restoration,the average bone absorption was 0.13±0.39 mm,the rate was 10.40±13.58%.After restoration,the MPBBM dropped down to 1.98±0.29 mm,the average bone absorption was 0.77±0.45 mm compared to pre-restoration,the rate was 26.5±13.51%.The average bone thickness of the labial side after restoration can meet the functional and aesthetic requirements.22 patients with 33 HBIC implants lacked the CBCT of two periods: immediately after surgery or before crown restoration.But the images after crown restoratio were available.the MPBBM was 2.56±0.37 mm.After crown restoration,the MPBBM decreased to 2.05±0.21 mm,the bone absorption rate was 18.22±14.45%.The average bone thickness of the labial side after restoration can satisfy the functional and aesthetic requirements.3.11 patients treated with paries lateralis approach of maxillary sinus floor lifting surgery by the CGF bone increment implanted a total of 29 HBIC implant.The CBCT images in three observation periods(immediately after surgery,during crown restoration,and after crown restoratio)were collected.The distance from the root of the implant to the maxillary sinus base wall were 5.29±0.95mm、4.78±0.85mm、4.44±1.22 mm.Compared with the postoperative outcomes,the bone absorption before restoration was 0.51±0.35 mm,the rate was 9.40±5.47%.After reparation,the average bone absorption was 0.21±0.25 mm,the percentage was 4.59±6.37%.The average bone height after restoration can satisfy the functional and aesthetic requirements.Conclusion: 1.CGF bone incremental technology can effectively increase the local bone mass in patients with bone deficiency.Postoperative bone graft material absorption should be taken into account as CGF bone incremental technology was performed.2.Non-invasive HBIC artificial dental implant can be used for simultaneous bone increment during implantation,and the clinical effect is reliable.
