| Objective : To study level changes of Th17/Treg lymphocyte subpopulations and their corresponding cytokines IL-17,IL-6,transforming growth factor-β(TGF-β),IL-10 and IL-1β in the blood of gestational diabetes mellitus(GDM)patients and to discuss about their correlations with GMD.Method: 96 women identified positive in the pregnancy test in the Zibo First Hospital from May,2017 to May,2018 were selected as study objects.Oral glucose tolerance test(OGTT)was carried out for all study objects,and according to test results,they were divided into two groups: gestational diabetes mellitus(GDM)group and non-GDM group.In addition,40 healthy females accepting physical examination in our hospital were selected as the control group.Flow cytometry(FCM)was used to detect Th17/Treg lymphocyte subpopulation distribution in the blood of patients in different groups;enzyme-linked immunosorbent assay(ELISA)was used to detect levels of IL-17,IL-1β,IL-10,TGF-β,IL-6 and C-reactive protein(CRP)in the serum as well as contents of fasting blood glucose(FBG),postprandial blood glucose(1h PBG and 2h PBG),total cholesterol(TCH),triglyceride(TG)and low-density lipoprotein(LDL).Insulin resistance index(HOMA-IR)was calculated,and difference and correlation between different groups in detected data were analyzed.Results:(1)Blood I L-17 level in the GDM group was obviously higher than those in non-GDM group and control group,and the differences had statistical significances(P<0.01).Serum IL-17 level in non-GDM group was higher than that in the control group,and the difference had statistical significance(P<0.05).Serum IL-1β level in GDM group was higher than that in non-GDM group with their difference presenting a statistical significance(P<0.05).IL-1β level in GDM group was obviously higher than that in the control group,and their difference had statistical significance(P<0.01),and that in non-GDM group was higher than that in the control group,and their difference also had statistical significance(P<0.05).Serum IL-10 level in GDM group was lower than that in non-GMD group but higher than that in the control group,and both differences had statistical significances(P<0.01).(2)FBG,1h PBG,2h PBG,TG and TCH test results indicated that: their expression levels in GDM group were higher than those in non-GDM group,and differences had statistical significances(P<0.05).HOMA-IR level in GDM group was significantly higher than that in non-GDM group,and the difference had statistical significance(P<0.01).GDM group and non-GDM group had no statistical differences in the aspect of LDL results(P>0.05).Comparative differences between non-GDM group and control group had no statistical significances(P>0.05).(3)Pearson analysis showed that IL-17 and IL-1β presented obvious positive correlations with 1h PBG,2h PBG,TG and HOMA-IR(P<0.01);IL-10 presented obvious negative correlations with HOMA-IR,FBG,2h PBG and 1h PBG(P<0.01),and IL-17,IL-1β,TCH and IL-10 had no correlations with TCH or TG(P>0.05).(4)Linear regression analysis,which took HOMA-IR value as dependent variable and IL-17,IL-1β and IL-10 as independent variables,showed that: IL-17 and IL-1β presented obvious positive correlations with HOMA-IR(P<0.01),and IL-10 presented obvious negative correlation with HOMA-IR(P<0.01).(5)The linear regression,which took TG as dependent variable and L-17,IL-1β and IL-10 as independent variables,showed that: IL-17 and IL-1β contents presented obvious positive correlation with TG(P<0.01),and IL-10 had no correlation with TG(P>0.05).(6)CRP and IL-6 levels in the serum in GDM group were obviously higher than those in non-GDM group and control group,and both differences had statistical significances(P<0.01),and those in non-GDM group were obviously higher than those in the control group,and both differences had statistical significances(P<0.01).TGF-β level in the serum in GDM group was lower than that in non-GDM group but higher than that in the control group,and both differences had statistical significances(P<0.05);TGF-β level in non-GDM group was higher than that in the control group,and the difference had statistical significance(P<0.01).(7)Correlation analysis of CRP content with IL-6,IL-17 and TGF-β showed that: CRP content in GDM group was obviously correlated with IL-7 and IL-6(P <0.05),and CRP content had obvious negative correlation with TGF-β(P <0.05).(8)Th17/Treg lymphocyte distribution results in the peripheral blood in different groups through FCM showed that Th17 lymphocyte in the GDM group was obviously higher than those in the non-GDM group and control group,and the results were of statistical significances(both satisfying P<0.01),and that in non-GDM group was higher than that in the control group,and the difference had statistical significance(P<0.05);Treg lymphocyte in the GDM group was obviously lower than those in the non-GDM group and control group(P<0.01)and that in the non-GDM group was obviously higher than that in the control group,and the difference had statistical significance P<0.01).Conclusion:(1)Disorder of Th17/Treg related cytokines gives rise to a series of pathological changes to GDM patients,e.g.impaired insulin resistance and glucose tolerance and lipid metabolism disorder.(2)Imbalance of Th17/Treg cells exists in GDM patients,which may be one of pathogeneses for GDM. |
