Objective In view of the existing problems in the development of secondary general hospitals under the current situation,this paper puts forward effective strategies for the development of secondary hospitals relying on the medical alliance to provide policy recommendations for further enhancing and strengthen in the medical alliance the important role of secondary hospitals in the tertiary medical service network.Methods Taking a secondary general hospital in the medical alliance as the main research object.Through the evaluation index system of the operation effect of the medical association,including business level,service efficiency and patient satisfaction,to evaluate the operation effect of the medical association.(1)Taking the basic situation,business level,service efficiency and other indicators of secondary hospitals from 2015 to 2017,descriptive analysis were carried out by using SPSS25.0 statistical software to understand the medical profession.The operation of the secondary hospital after the establishment of the Federation.(2)The inpatients before the establishment of the medical association,half a year after the establishment and one year after the establishment of the Medical Association were surveyed by telephone to complete the questionnaire.The comprehensive average score of satisfaction was calculated and the variance analysis was carried out to compare the differences of satisfaction.Using binary logistic regression analysis,the factors affecting satisfaction after the establishment of the Medical Association were further studied.(3)SWOT analysis was used to analyze the advantages,disadvantages,opportunities and challenges of the development of secondary hospitals,and put forward some strategic suggestions for the development of secondary hospitals.Result:The development status of secondary hospitals is limited in medical technology,unable to meet the medical needs,health resources shortage,resulting in limited development.After the establishment of medical alliance,the development of secondary hospitals has achieved remarkable results.During the three years from 2015 to 2017:(1)The number of outpatient visits in secondary hospitals rose from 329,086 to 405,532,an increase of 23.23%;the number of emergency cases rose from 108,101 to 178,703,an increase of 65.31%;and the number of hospitalizations rose from 16,227 to 20,512,an increase of 26.41%.(2)The service efficiency index of secondary hospitals: the number of bed turnover increased from 19.3 to 23.6;the average length of stay decreased from 7.6 days to 7.5days;the actual number of total open beds increased from 74,694 to 82,800.(3)After the establishment of the Medical Consortium,compared with the pre-establishment,the satisfaction of discharged patients increased with statistical significance(p < 0.05).Further analysis of the influencing factors of satisfaction was made.In terms of eight factors,such as medical service,medical technology,nursing service,nursing technology,toll office,pharmacy and ultrasound,there was statistical significance in the overall satisfaction of discharged patients,among which medical technology had a significant impact on the overall satisfaction.The impact weight is the largest.Conclusion:After the establishment of the medical alliance,the service ability of the secondary hospitals has been significantly enhanced,the medical efficiency has been significantly improved,and the satisfaction of the service objects has been greatly improved.However,there are still many problems in the development of secondary hospitals in the medical alliance,such as insufficient policy orientation,unclear function orientation and unbalanced resource allocation.To this end,the following policy recommendations are put forward:(1)grasp the policy direction,conform to the tide of reform,to organize and manage scientifically;(2)standardize technical guidance,smooth referral channels to optimize the division of labor and cooperation;(3)give full play to the advantages of talent,ensure the continuity of cooperation to take into account the interests of all parties.
